LI Chunting's Experience in Treating Gastric Cancer

摘　　要：李春婷认为胃癌为患者久病,气血亏虚,体质羸弱,癌毒伺机入侵而发病,其发生、发展是因虚致实,病位在脾胃。脾虚为发病根本,治疗益气健脾;久病气耗,或情志内伤,进而胃阴亏耗,治宜益胃养阴;脾喜燥恶湿,脾虚湿浊蕴久化痰,与病情进展关系密切,影响津血运行而化瘀,治以涤痰化瘀;肝助胃和降,患者情志不舒,治以疏肝理气、养血柔肝;癌毒内犯者,毒瘀交结,以解毒散结为要务。根据患者综合情况,辨病与辨证结合,治疗上则治本与治标各有主次,虚实兼顾。治本以健脾益气、益胃养阴;治标则根据致病因素以涤痰化瘀、疏肝和胃、抗癌解毒。从中医整体观念出发,因人而异,补泄有度。LI Chunting believed that the occurrence and development of gastric cancer were the process of the deficiency causing excess syndrome. Chronic disease leading to Qi and blood deficiency and physical weakness while cancerous pathogen invaded to cause disease. Therefore,spleen deficiency affected the occurrence,development and prognosis of gastric cancer which was located in the spleen and stomach. Spleen deficiency is the root of gastric cancer,so replenishing Qi and invigorating the spleen should be the treatment principle. Long illness resulting in Qi consumption or internal emotional impairment damaging Yin causing stomach Yin deficiency should be treated by nourishing the stomach Yin. Spleen liking dryness and disliking dampness,so the spleen deficiency and dampness transforming into phlegm were close related with disease progression,even affecting the circulation of blood and body fluid resulting in blood stasis which should be treated by eliminating the phlegm and blood stasis. Liver help the descending of the stomach,patients with gastric cancer had emotional discomfort which should be treated by soothing the liver,regulating Qi,nourishing blood and softening the liver. Cancerous pathogen invaded and combined with blood stasis which should be treated by eliminating the cancerous pathogen and dispersing the mass. Professor LI summarized the clinical experience by combination of syndrome differentiation and disease differentiation,believed that treating the root and branch according to patient＇s comprehensive situations. Treating the root referred to invigorate the spleen,replenish Qi and nourish the stomach Yin. Treating the branch referred to eliminate the phlegm,blood stasis,soothe the liver to harmonize the stomach,remove the cancerous pathogen to resist cancer. Holism and individual constitutions should be considered in treating gastric cancer.
