双环醇联合阿德福韦酯治疗 HBeAg 阳性慢性乙型肝炎疗效观察

摘　　要：目的：观察双环醇联合阿德福韦酯治疗 HBeAg 阳性慢性乙型肝炎疗效和安全性。方法选择118例 HBeAg 阳性慢性乙型肝炎患者，随机分为两组。对照组58例，口服阿德福韦酯胶囊10 mg/次、1次/ d；治疗组60例，在对照组治疗基础上口服双环醇片25 mg/次，3次/ d。两组均不用其他抗病毒、免疫调节及抗肝纤维化药物，疗程共48周。观察乙肝病毒学指标（HBV DNA、HBeAg 阴转率、HBeAg 血清学转换率）、肝功能、肝纤维化指标的变化。结果疗程结束时，两组 HBV DNA 低于检出下限比率，HBeAg 阴转率、HBeAg 血清学转换率均相似，差异未见统计学意义（P 均﹥0.05）；治疗组患者肝功能改善和肝纤维化指标下降程度均优于对照组，差异均有统计学意义（P ﹤0.01，P ﹤0.05）。两组均无药物不良反应发生。结论双环醇联合阿德福韦酯治疗 HBeAg 阳性慢性乙型肝炎安全、有效，在改善肝功能及肝纤维化指标方面疗效明显优于单用阿德福韦酯。Objective To compare the efficacy and safety of bicyclol combined with adefovir dipivoxil in the treat-ment of HBeAg positive chronic hepatitis B. Methods The 118 patients with HBeAg positive chronic hepatitis B were ran-domly divided into treatment group（60 cases）and control group（58 cases）. For the control group,each patient was given adefovir dipivoxil 10 mg,1 time/ d,take orally. For the treatment group,on the basis of control group,each patient was given bicyclol tablets 25 mg,3 times/ d,take orally. No other drugs of anti-virus,immunity adjusting and anti-hepatic fi-brosis were used in both groups,the treatment lasted for 48 weeks. The assessments of hepatitis B virus index（HBV DNA, HBeAg-negativity,HBeAg seroconversion）,liver function and liver fibrosis index were performed. Results At the end of the treatment,the rates of HBV DNA below detection limit,HBeAg-negativity and HBeAg seroconversion of the two groups were similar,there was no significant difference（P ﹥ 0. 05）;but there were significant differences in liver function and re-duction of liver fibrosis index between the treatment group and control group（P ﹤ 0. 01,P ﹤ 0. 05）. There were no adverse drug reactions. Conclusions Bicyclol combined with adefovir dipivoxil in the treatment of HBeAg positive chronic hepati-tis B is safe and effective. The efficacy is striking better than that of only use of adefovix dipivoxil.
