摘 要:二肽基肽酶4(DPP-4)抑制剂的降糖作用已非常明确,其主要通过抑制DPP-4酶的活性,减少肠促胰素降解,从而增加体内胰高血糖素样肽1和葡萄糖依赖性促胰岛素激素水平,促进胰岛素分泌以及抑制胰高血糖素分泌,并且降低胃排空率等来降低血糖。研究显示,DPP-4抑制剂具有心血管系统保护作用,其机制可能是通过降低血脂和血压、减轻炎症反应和氧化应激以及维持血管内皮细胞的完整性来实现的。DPP-4抑制剂是否能够改善心血管终点事件的发生危险尚无明确定论。It’s very clear that dipeptidyl peptidase-4( DPP-4) inhibitors decrease blood glucose mainly via reducing the degradation of incretins by DPP-4 enzyme,accordingly increase the levels of glucagon-like peptide-1( GLP-1) and glucose-dependent insulinotropic polypeptide( GIP). GLP-1 and GIP can improve the insulin secretion and inhibit the secretion of glucagon,as well as reduce the gastric emptying rate. Previous studies showed that DPP-4 inhibitors have important cardiovascular protective effects by reducing blood lipid and blood pressure,alleviating inflammation and oxidative stress,and supporting the integrity of the vascular endothelial cells. However,it is uncertain weather DDP-4 inhibitors can improve the risk of cardiovascular events.
