胸腔镜辅助小切口开胸术与全胸腔镜手术治疗自发性气胸临床效果比较

摘　　要：目的分析比较胸腔镜辅助小切口开胸术与全胸腔镜手术治疗自发性气胸的临床效果。方法选取2013年1月至2015年1月黄石市第二医院收治的自发性气胸患者84例,根据随机数字表法分为观察组和对照组,各42例。观察组接受胸腔镜辅助小切口开胸手术治疗;对照组接受全胸腔镜手术治疗。对两组患者的出血量、胸管引流时间和术后疼痛评分、并发症、复发率等进行记录与分析。结果观察组手术时间显著短于对照组[(55±16)min比(72±16)min],住院时间显著长于对照组[(6.4±1.1)d比(5.7±1.2)d],差异有统计学意义(P<0.05)。对照组总花费显著高于观察组[(18 064±476)元比(10 054±395)元],差异有统计学意义(P<0.05)。结论胸腔镜辅助小切口开胸术手术应用于自发性气胸不仅耗时短而且总经济费用低,值得在基层医院推广应用。Objective To compare clinical effect of thoracoscopy assisted small incision thoracotomy surgery and total thoracoscopic surgery on spontaneous pneumothorax. Methods Totla of 84 cases of spontaneous pneumothorax from Huangshi Second Hospital during Jan. 2013 and Jan. 2015 were selected,and these patients were divided to observation group and control group according to the random number table method,42 cases each group. The observation group was given thoracoscopy assisted small thoracotomy surgery,while the control group received total thoracoscopic surgery,and then the bleeding amount,chest tube drainage time and postoperative pain score,complications,recurrence rate of the two groups were recorded and analyzed.Results The operative time of the observation group was significantly shorter than the control group[（ 55 ±16） min vs（ 72 ± 16） min],and the hospital stay of the observation group was significantly longer than the control group[（ 6. 4 ± 1. 1） d vs（ 5. 7 ± 1. 2） d],the difference was statistically significant（ P 〈 0. 05）. The total cost of the control group was significantly higher than the observation group [（ 18 064 ± 476） yuan vs（ 10 054 ± 395） yuan],the difference was statistically significant（ P 〈 0. 05）. Conclusion Applying thoracoscopy assisted small incision thoracotomy surgery on spontaneous pneumothorax not only saves time,but also has lower total costs,which is worth applying in primary hospitals.
