人工髋关节置换手术治疗高龄股骨颈骨折临床分析

摘　　要：目的探讨人工髋关节置换手术治疗高龄股骨颈骨折的手术效果。方法选择2013年1月～2014年1月于我院治疗的60例高龄股骨颈骨折患者,全部患者随机均分为观察组（人工髋关节置换手术）与对照组（内固定术）,每组各30例,比较两组患者的术中出血量、手术时间及并发症情况,术后随访12个月,比较两组患者的髋关节功能优良率。结果观察组患者的手术时间短于对照组（P〈0.05）,观察组患者的术中出血量显著少于对照组（P〈0.05）。两组术后并发症发生率比较,差异具有统计学意义（10.0%vs 40.0%,P〈0.05）。术后随访12个月,观察组患者的髋关节功能优良率显著高于对照组,差异具有统计学意义（93.3%vs 70.0%,P〈0.05）。结论人工髋关节置换手术治疗高龄股骨颈骨折手术效果好,术中出血少、术后并发症少,且髋关节功能恢复好。Objective To investigate the effect of artificial hip replacement in the treatment of femoral neck fracture in the elderly. Methods From January 2013 to January 2014 in our hospital, 60 cases of elderly patients with femoral neck fracture patients were randomly divided to the observation group（artificial hip replacement surgery） and the control group（fixation）, 30 cases in each group, the intraoperative bleeding volume, operation time and complications were compared between two groups. 12 months postoperatively, hip joint function excellent and good rate were compared be tween two groups. Results The operation time of the observation group was shorter than that of the control group（P〈0.05）, the amount of bleeding in the observation group was significantly less than that in the control group（P〈0.05）. The inci dence of postoperative complications of the two groups was compared, the difference was significant between two groups（10% vs 40%, P〈0.05）. After 12 months follow-up, the excellent rate of hip joint function in the observation group was significantly higher than that in the control group, and the difference was significant（93.3% vs 70.0%, P〈0.05）. Conclusion Hip replacement surgery in the treatment of femoral neck fracture in elderly patients with good effect, less bleeding, postoperative complications, and hip function recovery is good.
