摘 要:目的探究血清和EPS中雌雄激素[雌二醇(E2)和睾酮(T)]水平与Ⅲ型前列腺炎患者勃起功能的相关性。方法选取2013年9月至2014年9月该院收治的372例Ⅲ型前列腺炎患者,按常规EPS检测结果将白细胞(WBC)≥10/HP的患者作为研究Ⅰ组(213例),WBC<10/HP的患者作为研究Ⅱ组(159例);再根据勃起功能国际指数(IIEF-5)分值对372例Ⅲ型前列腺炎患者进行分组,其中分值小于或等于21分的患者作为A组,大于21分的患者作为B组。另选取186例同期健康体检者作为对照组,采用酶联免疫吸附试验(ELISA)检测所有被研究者的血清和EPS中E2和T水平,并应用前列腺炎症状指数(NIH-CPSI)和IIEF-5对患者进行评分,分析其与血清和EPS中雌雄激素(E2、T)水平变化的关系。结果研究Ⅰ组和研究Ⅱ组的血清中E2水平及血清和EPS中E2/T的比值均明显高于对照组,而T水平显著低于对照组,差异均有统计学意义(P<0.05);研究Ⅰ组中共有99例患者被归入A组,较研究Ⅱ组的87例有所提高,但差异无统计学意义(χ2=0.824,P=0.364);A组患者的血清中T水平明显低于B组和对照组,而EPS中E2/T的比值则明显高于B组和对照组,另外EPS中E2水平也较B组明显升高,差异均有统计学意义(P<0.01);患者的IIEF-5评分与血清和EPS中T水平呈正相关的关系(r=0.137、0.145、P=0.031,0.022),与EPS中E2水平及血清和EPS中E2/T的比值呈负相关的关系(r=-0.132、-0.167、-0.152,P=0.038、0.008、0.017),而与NIH-CPSI评分之间不呈相关关系(r=0.025,P=0.695)。结论血清和EPS中E2和T水平的改变可影响Ⅲ型前列腺炎患者的勃起功能。Objective To explore the correlation between the change of estradiol and testosterone(E2,T)in serum and expressed prostatic secretion(EPS)with the erectile function in the patients with typeⅢ prostatitis(CP/CPPS).Methods 372 patients with typeⅢ prostatitis in our hospital from September 2013 to September 2014 were chosen and divided into the research groupⅠ(WBC≥10/HP,213cases)andⅡ(WBC〈10/HP,159cases)according to the test results of EPS,and then these 372 cases of typeⅢ prostatitis were divided into the group A(≤21points)and B(〉21points)according to the international index of erectile function-5(IIEF-5)scores.Other contemporaneous186 individuals undergoing the healthy physical examination were chosen as the control group.The levels of E2 and T in serum and EPS were detected by ELISA.NIH-CPSI index and IIEF-5index were applied to evaluate the patients.The correlation between the E2,T levels in serum and EPS with the erectile function was analyzed.ResultsThe E2 levels in serum and E2/T in serum and EPS of the research groupⅠ and Ⅱwere significantly higher than those in the control group,but the T level was lower than that in the control group,the differences were statistically significant(P〈0.05);99cases in the research groupⅠwere included in the group A,which were more than 87 cases in the research groupⅡ,but the difference was not statistically significant(χ2=0.824,P=0.364);the serum T level in the group A was significantly lower than that in the group B and the control group,but the E2/T ratio in EPS was significantly higher than that in the group B and the control group,moreover,the E2 level in EPS was significantly increased compared with the group B,the difference was statistically significant(P〈0.01);the IIEF-5scores had the positive correlation with the T level in serum and EPS(r=0.137,0.145,P=0.031,0.022),and had the negative correlation with the E2 level in EPS and the E2/T ratio in serum and EPS(r=-0.132,-0.167,-0.152,P=0.038
