摘  要:目的:探讨连续性血液滤过(CHF)对重症中暑患者内环境和生存率的影响。方法:2011-07-2013-08因重症中暑收入我院GICU患者42例,分为对照组(n=22)和研究组(n=20),观察2组患者体温、炎性递质及病死率情况。结果:2组治疗后1、6、24、48h体温下降比较,差异均有统计学意义(P<0.05)。治疗48h后,研究组炎性递质肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)显著降低,与对照组比较差异均有统计学意义(P<0.05)。结论:CHF能够改善重症中暑患者的预后,延长其存活时间,改善患者的生存率,其机制可能与早期快速降温,及时清除炎性递质有关。Objective:To explore the effect of continuous hemofiltration on internal environmental and survival rate of patient with severe heatstroke treated with continuous hemofiltration(CHF).Method:The severe heatstroke patients admitted to GICU during 2011.07 to 2013.08 were divided into control group(n=22)and research group(n=20).We detected their temperature,inflammatory cytokines and survival rate in the study.Result:There was a statistically significant difference on central temperature between research group and control group at 1hour,24 hours and 48 hours after therapy(P〈0.05).And there was a statistically difference of inflammatory cytokines clearance in TNF- α、IL-6、IL-8between control group and research group(P〈0.05).Conclusion:The CHF treatment can improve the outcome of severe heatstroke patients.It can prolong the survival time,reduce the mortality.The possible mechanism is related to inflammatory cytokines clearance and hypothermia induced by CHF quick hypothermia in early period.
