关节镜手术与非手术治疗踝关节退行性骨关节病的疗效比较

摘　　要：目的探讨关节镜手术与非手术治疗踝关节退行性骨关节病的疗效比较。方法选择并纳入我院2013年1月~2014年2月踝关节退行性骨关节病患者68例进行分组研究。手术组：采用关节镜手术治疗;非手术组：采用踝关节退行性骨关节病保守治疗。对比两组患者治疗优良率、治疗前后VAS评分、Tegner评分、Mazur评分变化。结果手术组治疗优良率跟非手术组接近,差异无统计学意义（P〉0.05）。治疗前两组患者VAS评分、Tegner评分、Mazur评分相似,差异无统计学意义（P〉0.05）;治疗后,两组患者VAS评分、Tegner评分、Mazur评分均明显优于治疗前,差异有统计学意义（P〈0.05）。治疗后两组患者VAS评分、Tegner评分、Mazur评分相似,差异无统计学意义（P〉0.05）。结论关节镜手术与非手术治疗踝关节退行性骨关节病均有一定疗效,在改善患者疼痛症状、踝关节功能等方面均有良好效果,可根据患者实际情况选择相应的治疗方法。Objective To compare the efficacy between arthroscopic surgery therapy and non-operative therapy in the treatment of ankle degenerative osteoarthropathy. Methods 68 patients with ankle degenerative osteoarthropathy who were in our hospital from January 2013 to February 2014 were divided into operation group and non-operation group. The operation group received arthroscopic surgery treatment and the non-operation group received conservative treatment. The excellent-effective rates as well as the VAS scores, Tegner scores, and Mazur scores before and after treatment of the two groups were compared. Results The excellent-effective rate of the operation group was close to that of the non-operation group, there was no statistical significance（P 0.05）. Before treatment, the VAS scores, Tegner scores, and Mazur scores of the two groups were close to each other group, the differences were not statistically significant（P 0.05）. After treatment, the VAS scores, Tegner scores, and Mazur scores of the two groups were obviously better than those before, the differences were statistically significant（P 0.05）, and three kind of scores of the two groups were close to each other group, there was no statistical significance（P 0.05）. Conclusion In the treatment of ankle degenerative osteoarthropathy, the arthroscopic surgery therapy and non-operation therapy are both effective, and can relieve the pain symptoms and improve the ankle functions of patients. In clinical practice, among the two treatment methods, the appropriate one should be chosen according to the actual situation of patients.
