青岛市糖尿病危险评分筛查代谢综合征的效果评估

摘　　要：目的使用青岛市糖尿病危险评分表构建用于代谢综合征（MS）发生风险的预测工具,为代谢综合征的早期筛查方法、干预措施的制定提供科学依据。方法随机抽取青岛地区7 500名35~75岁常住居民作为研究对象进行调查,以青岛糖尿病危险评分值为自变量,以是否患有代谢综合征为结局变量绘制ROC曲线,计算灵敏度和特异度,并根据最大约登指数判断最佳切点。结果青岛市糖尿病危险评分筛查代谢综合征的最佳切点为14.50,曲线下面积（AUC）为0.77（95%CI0.75~0.78）,灵敏度为73.67%,特异度为68.42%。其中男性的最佳切点为14.50,AUC为0.76（95%CI0.74~0.78）,灵敏度为72.13%,特异度为69.28%;女性的最佳切点为15.50,AUC为0.77（95%CI0.75~0.78）,灵敏度为74.68%,特异度为68.24%。青岛糖尿病危险评分表筛查代谢综合征的AUC明显高于腰高比。结论青岛市糖尿病危险评分筛查代谢综合征的准确性较高,是开展社区筛查和健康促进的最佳模型。Objective To utilize Qingdao diabetes risk score form as a prediction tool for metabolic syndrome（MS）,so as to provide scientific basis for screening method or intervening measure on metabolic syndrome. Methods A total of 7500 subjects aged 35-74 years were randomly selected and surveyed.Qingdao diabetes risk score was used for independent variable;incidence of metabolic syndrome as outcome variable was used for drawing ROC curve and for calculating sensitivity and specificity;then the optimal cut-off value was evaluated by Youden＇s index. Results The optimal cut-off value of Qingdao diabetes risk score for metabolic syndrome was 14.50.The area under the curve（AUC）was 0.77（95%CI ：0.75-0.78;Sensitivity：73.67%;Specificity：68.42%）.The optimal cut-off value among male was 14.50.The AUC was0.76（95% CI ：0.74-0.78;Sensitivity：72.13%;Specificity：69.28%）.The optimal cut-off value among female was15.50.The AUC was 0.77（95%CI ：0.75-0.79;Sensitivity：74.68%;Specificity：68.24%）.The AUC of Qingdao diabetes risk score for screening metabolic syndrome was significantly higher than waist to height ratio. Conclusion The accuracy of Qingdao diabetes risk score for screening metabolic syndrome is high,which is a good model for carrying out community screening or health promotion.
