摘 要:目的探讨壹期矽肺患者血清中巨噬细胞炎症蛋白-1α(MIP-1α)、巨噬细胞炎症蛋白-1β(MIP-1β)和巨噬细胞炎症蛋白-3α(MIP-3α)水平变化及其与肺功能关系。方法用Luminex流式荧光技术检测55例壹期矽肺(矽肺组)、35例具有与矽肺组相同接尘条件的健康井下接尘矿工(接尘组)和33例非接尘井上健康人员(对照组)血清中MIP-1α、MIP-1β和MIP-3α水平;用肺功能机检测用力肺活量(FVC%)、1秒钟用力呼气量(FEV_(1.0)%)、用力呼气1秒率(FEV_(1.0)/FVC%)、最大呼气中期流速(MMEF 75/25%)4项肺功能指标。结果与对照组比较,接尘和矽肺组血清中MIP-1α水平均明显降低,接尘组MIP-1β水平也明显降低,差异均有统计学意义(均P<0.01);与接尘组比较,矽肺组血清中MIP-1α和MIP-1β水平均显著升高,差异均有统计学意义(均P<0.01);吸烟矽肺患者血清中MIP-1β水平低于非吸烟矽肺患者,差异有统计学意义(P<0.05);矽肺组4项肺功能指标明显低于接尘组,差异均有统计学意义(均P<0.01);矽肺组MIP-1α与FVC%、MIP-1α与FEV_(1.0)%均呈正相关(r=0.330、0.324,P<0.05)。结论壹期矽肺患者血清中MIP-1α和MIP-1β水平异常,二者参与壹期矽肺发生发展;吸烟影响MIP-1β水平;MIP-1α水平可间接反映肺功能。[Objective] To explore the relationship between serum levels of macrophage inflammatory protein1α (MIP-1α), macrophage inflammatory protein 1β( MIP- 1β), macrophage inflammatory protein 3α( MIP-3α) and lung function in patients with first-stage silicosis. [Methods]Serum levels of MIP-1α, MIP-1β and MIP-3α were determined in 55 cases of first-stage silicosis (silicosis group), 35cases of healthy miner(dust-exposed group) and 33 cases of healthy control(control group) by Luminex 200. The forced vital capacity (FVC%), forced expiratory volume in one second (FEV1.0%), FEV1.0/FVC%, and maximal mid- expiratory flow (MMEF 75/25% ) were determined by special lung function machine.[Results] The levels of serum MIP-1α in dust-exposed group and silicosis group were significantly lower than that in control group, while the level of serum MIP-1β in dust-exposed group was significantly lower than that in control group, and the differences were statistically significant (all P〈0.01 ). The levels of serum MIP-1α and MIP-1β in silicosis group were significantly higher than those in dust-exposed group, and the differences were statistically significant (both P〈0.01 ). The level of serum MIP-1β in smoking patients with silicosis was significantly lower than that in never-smoking patients with silicosis, and the difference was statistically significant (P〈0.05). The levels of four lung function indexes in silicosis group were significantly lower than those in dust-exposed group, and the differences were statistically significant (all P〈0.01 ). Additionally, there was a positive correlation between MIP-1α level and FVC%, MIP-1α level and FEV1.0% in silicosis group (r=0.330, r=0.324, P〈0.05 ). [Conclusion] The levels of serum MIP-1α and MIP-1β are abnormal in patients with first-stage silicosis, and they are associated with the occurrence and development of silicosis.
