术前镇静药对老年患者NI的影响

摘　　要：目的探讨不同术前镇静药对老年患者Narcotrend指数（NI）的影响。方法行宫腔镜检查手术的老年患者60例,随机分为咪达唑仑组（M组）、鲁米那组（L组）和对照组（C组）3组（n=20）,术前30min肌肉注射咪达唑仑2mg（M组）、鲁米那0.1 g（L组）和生理盐水0.5 ml（C组）,麻醉诱导用药为静脉注射芬太尼0.1 mg,丙泊酚靶控输注血浆效应室浓度为2.5-3.5μg/ml。术中持续泵入丙泊酚,术毕迅速停药。分别记录患者入室时（T1）、麻醉诱导后（T2）、手术开始时（T3）和苏醒时（T4）的NI、心率（HR）、平均动脉压（MAP）,以及麻醉停药到苏醒的时间。结果在T1时间点,M组和L组的NI、HR、MAP比C组的NI、HR、MAP数值小（P〉0.05）。在T2、T3、T4 3个时间点,3组的NI、HR、MAP相比较差异无统计学意义（P〉0.05）。3组患者麻醉停药到苏醒的时间相比较差异无统计学意义（P〉0.05）。结论术前使用不同镇静药（咪达唑仑和鲁米那）可降低老年患者的NI数值。Objective To discuss the effect on the different preoperative sedatives in Narcotrend Index（NI）of elderly patient.Methods Sixty elderly patients who had the hysteroscopy were randomly divided into a midazolam group（M group）,a luminal group（L group）and a control group（C group）（n=20）,by intramuscular injection in midazolam2 mg（M group）,luminal 0.1 g（L group）and NS0.5 ml（C group）in 30 minutes before operation.Anesthesia induction drugs were fentanyl 0.1 mg by intravenous injection and propofol by Cp 2.5~3.5μg/ml of TCI.Propofol was continual intravenous injection during the operation and was rapidly stopped when the operation was finished.NI,HR and MAP were measured in when the patient entered into the operating room（T1）,after the anesthesia induction（T2）,when the operation was started（T3）,and the patient was awake（T4）.The time from the anaesthetic stopped to the patient awake was measured too.Results NI,HR and MAP in M group and L group were significantly decreased lower than those in C group in T1（P〈0.05）.NI,HR and MAP in M group had no significantly difference comparing with L group in T1（P〉0.05）.NI,HR and MAP had no significantly difference among 3 groups in T2,T3 and T4（P〉0.05）.The time from the anaesthetic stopped to the patient awake had no significantly difference among 3 groups（P〉0.05）.Conclusion The different sedatives（midazolam and luminal）used before the operation can reduce the NI of elderly patient.
