下肢慢性溃疡的修复治疗

摘　　要：目的提高下肢慢性溃疡修复治疗水平。方法对37例下肢慢性溃疡患者进行封闭式可调节负压吸引术(RNPT)联合手术治疗(皮肤移植术或皮瓣转移术)。结果 37例患者均接受RNPT联合手术治疗后,绝大部分患者创面形态及所在部位功能均获得良好的改善,创面愈合,患者及其家属满意,1例糖尿病足患者因病情较重,在行RNPT治疗后创面持续溃烂,大量组织腐败坏死,因经济条件有限,最终在患者本人及家属要求下对患者患足行截肢。结论 RNPT技术操作简单,可减少医护人员对下肢慢性溃疡患者治疗的工作量,提高治疗效果。Objective To raise the level of repair treatment of chronic ulcers of the lower limbs. Methods Thirty-seven patients with chronic ulcers of the lower limbs received regulated negative pres- sure therapy （RNPT）after surgical treatment（skin grafting or skin flap transposition）. Results After the treatment of RNPT and surgical treatment, the wound and related functions were repaired and improved in the 37 patients. Most of thepatients and their families were satisfied with the outcomes, but one patient with serious diabetic foot had festered wound and tissue necrosis after RNPT. Due to the economic limitation, the patienteventually had his lower limb amputated at the request of himself and his families. Conclusion RNPT is characterized by simple operation and good effectiveness ,which reduce the workload of medical workers for chronic ulcers of the lower limbs.
