肺结核合并肺曲霉菌感染的CT特征分析

摘　　要：目的探讨肺结核合并曲霉菌感染的CT表现,提高诊断准确性。方法回顾性分析2013年1月—2015年1月收治的11例肺结核合并曲霉菌感染患者的临床资料及CT检查图像资料,观察CT影像表现。结果病灶分布:双侧肺8例,单侧肺3例;多叶10例,单叶1例。病灶形态:多发斑片状、小结节状影,呈"树芽征"、"晕征"共4例;片状实变影5例;空洞内随体位移动软组织结节灶7例,空洞内条索状、絮状内容物4例;伴有胸腔积液和(或)胸膜肥厚6例。结论肺结核合并曲菌感染CT影像表现具有特征性,空洞内曲菌球及絮状、条索状内容物形成有重要的诊断价值。Objective To study the CT manifestation of tuberculosis complicated by aspergillus infection,in order to improve the diagnostic accuracy of pulmonary tuberculosis complicated by aspergillus infection. Methods A retrospective analysis from January 2013 to January 2015,the clinical data of 11 patients with tuberculosis complicated by aspergillus infection and CT image data,to observe the CT imaging findings. Results Distribution of lesions placed in bilateral lungs was 8 cases,unilateral lung（n=3）, multilobe lungs（n=10）,single lobe（n=1）.Lesions form：multiple patchy,nodular shadows, ＂tree-in-bud＂, ＂halo sign＂ were in 4 cases;Patchy consolidation shadows were in 5 cases;cavities with the position changed soft tissue nodules in 7 cases,funicular,flocculent content within cavities were 4cases,Accompanied with pleural effusion and/or pleura hypertrophy were 6 cases. Conclusion Imaging findings of pulmonary tuberculosis complicated by aspergillus infection has characteristic,aspergilloma and Flocculent within the cavity has important diagnostic value.
