摘　　要：目的评估东营市手足口病经济负担。方法将全市所有县级及以上医院作为调查现场,抽样调查2014年6-8月份就诊的手足口病普通门诊、普通住院、重症病例,收集患者就医的直接医疗费用、直接非医疗费用和间接费用,估算手足口病经济负担。结果共调查门诊病例89例,普通住院病例90例,重症病例50例。估计2014年东营市手足口病总经济负担达1 108.67万元。门诊病例、普通住院病例、重症病例的例均总经济负担分别为835.48元、3 870.50元、14 549.28元,例均直接经济负担分别为469.30元、2 811.11元、12 999.11元,例均间接经济负担分别为366.18元、1 059.39元、1 550.16元。患者就诊于级别高的医院,直接经济负担相应增加。城镇患者的间接经济负担明显高于农村患者。结论手足口病给患者和社会均造成较重的经济负担。采取"三早"防控策略,强化基层卫生服务体系建设,规范患者入院标准,提高重症病例医疗费用补偿比例等措施将有助于减轻患者经济负担。Objective To estimate the economic burden caused by hand- foot- mouth disease（ HFMD） in Dongying. Methods The data between June 2014 and August 2014 on direct medical cost,direct non- medical cost and indirect cost of sampling surveyed HFMD outpatients,general and severe inpatients in Dongying was collected. Results A total of 89 outpatients,90 general inpatients and 50 severe inpatients of HFMD were surveyed. The overall economic burden attributable to HFMD was 11.0867 million yuan in Dongying in 2014. The average economic burden of outpatients,general and severe inpatients were 835.48,3870. 50 and 14549. 28 yuan,respectively. The mean direct economic burden were 469. 30,2811. 11 and 12999. 11 yuan,respectively. And the mean indirect economic burden was 366. 18,1059. 39 and 1550. 16 yuan,respectively. If the patients see a doctor in a high- level hospital,the direct economic burden would increase. The indirect economic burden of patients in town was obviously higher than that of patients in the countryside. Conclusion The economic burden of HFMD was heavy both for the society and for patients. Those measures that using the ＂three early＂prevention and control strategy,strengthening the construction of health service system,standardizing patients admitted to hospital and improving the medical cost compensation proportion of severe inpatients would lighten the economic burden of HFMD patients.
