摘 要:目的:评价骨疏康治疗原发性骨质疏松症的疗效及安全性。方法:通过检索CNKI、CBM、维普、万方等中文生物医学数据库,收集关于骨疏康治疗原发性骨质疏松症随机对照试验的文献,检索文献的日期从建库以来至2013年10月。采用Cochrane系统评价的方法,按照纳入及排除标准提取资料,以Detsky评估文献质量并提取有效数据后,用Rev Man 5.2进行Meta分析。结果:共纳入以骨疏康或骨疏康加钙剂或加维生素D为干预措施进行的临床随机对照研究21篇,其中高质量2篇,中质量19篇,总计1352例。在提高骨密度、改善骨痛、维持血清磷、碱性磷酸酶、雌激素正常水平方面,骨疏康组与对照组治疗原发性骨质疏松症的疗效相当,差别无统计学意义(P>0.05);而在维持血钙方面,则骨疏康组与对照组有差异(P<0.05)。骨疏康不良反应发生率低于对照组,但差别无统计学意义(P>0.05)。结论:骨疏康可提高原发性骨质疏松症的骨密度、改善骨痛情况,与其他抗骨质疏松药物疗效相当,且在维持正常血钙水平方面优于其他抗骨质疏松药物,而在改善血清磷、碱性磷酸酶及雌激素水平方面则效果相当,且存在一定的不良反应,临床应用时适当注意。Objective:To evaluate the efficacy and safety of Gushukang for the treatment of primary osteoporosis. Methods:By searching Chinese biomedical database such as CNKI,CBM,CQVIP,WANFANGDATA,we collected literatures of random clinical trials using Gushukang for treatment of primary osteoporosis. The dates of the literatures searched started from the beginning of the database to October 2013. Using Cochrane systematic review,we retrieved data according to the inclusion and exclusion criteria. We further used Detsky to evaluate the quality of the literature and retrieve effective data,and finally applied Rev Man 5.2 for meta-analysis. Results:21 papers of randomized controlled clinical trials about Gushukang,or Gushukang plus calcium supplementation,or Gushukang plus vitamin D in treating primary osteoporosis were included,of which 2 papers were high quality,19 papers were medium quality. The total cases in analysis were 1352. There are no significant differences in improving BMD,releasing bone pain,and maintaining the serum phosphate,ALP and serum estrogen levels between Gushukang and control groups(P〈0.05). But it shows a significant difference in maintaining serum calcium level between Gushukang and control groups(P〈0.05). The incidence of adverse effects in Gushukang group is lower than control group,but it's not significant(P〈0.05). Conclusion:Gushukang can improve the BMD and release bone pain of primary osteoporosis,which is the same efficacy with the other anti-osteoporosis drugs. Gushukang has better effect than the other anti-osteoporosis drugs in maintaining the normal serum calcium level. The effects of Gushukang and the other anti-osteoporosis drugs are the same in improving serum phosphate,ALP and serum estrogen levels,and all of them have certain adverse effects. We should pay more attention to the adverse effects when applying Gushukang in clinic.
