103例重症手足口病患儿脑脊液分析

摘　　要：目的分析重症手足口病患儿脑脊液情况,探讨其脑脊液特点及临床意义。方法整群收集2013年3月—2014年6月在郑州市儿童医院住院的103例重症手足口病脑脊液标本,比较重型手足口病及危重型手足口病脑脊液差异。结果危重型手足口病患儿脑脊液的白细胞数（85.15±81.24）×10^6/L,细胞数升高比例77.27%（17/22）,分别高于重症手足口病的（75.33±68.52）×10^6/L、44.44%（36/81）,差异有统计学意义（P〈0.05）。危重型手足口病的脑脊液蛋白定量（0.89±0.81）g/L,蛋白升高比例77.27%（17/22）,分别高于重症手足口病的（0.68±0.45）g/L、51.85%（42/81）,差异有统计学意义（P〈0.05）。结论脑脊液检查结果作为评价手足口病患儿中枢神经系统病变的指标,在预警危重型手足口病方面有一定临床意义。Objective To analyze the characteristics and clinical significance of cerebrospinal fluid（CSF） in children with severe hand-foot-and-mouth disease. Methods The CSF specimens of 103 children with severe hand-foot-and-mouth disease hospitalized in Children＇s Hospital of Zhengzhou from March 2013 to June 2014 were collected. And the CSF was compared between the severe and critical severe hand-foot-and-mouth disease children. Results Compared with the children with severe hand-foot-and-mouth disease, the children with critical severe hand-foot-and-mouth disease had much higher white blood cell count [（85.15±81.24） ×10~6/L vs（75.33±68.52） ×10~6/L], obviously higher ratio of cells rising[77.27%（17/22） vs 44.44%（36/81）], the differences were statistically significant（P〈 0.05）. Compared with the children with severe hand-foot-and-mouth disease, the children with critical severe hand-foot-and-mouth disease had much higher protein quantity in CSF[（0.89±0.81） g/L vs（0.68±0.45） g/L], and obviously higher ratio of protein levels increase[77.27%（17/22） vs51.85%（42/81）], the differences were statistically significant（P〈 0.05）. Conclusion As the evaluation index of the central nervous system lesions in children with hand-foot-and-mouth disease, CSF examination is of certain clinical significance in the prediction of critical severe hand-foot-and-mouth disease.
