摘 要:目的:应用大鼠酸吸入急性肺损伤模型,在酸吸入损伤后早期吸入七氟醚,观察对大鼠急性肺损伤NF-кB活性的影响及肺保护作用。方法:随机将SD大鼠分为5组(n=16),生理盐水组:气管内注入生理盐水1m L/kg;盐酸吸入组:气管内注入p H=1.25盐酸溶液1m L/kg;七氟醚处理组:气管内注入盐酸溶液,并分为3个亚组(七氟醚0.5h组,七氟醚1h组,七氟醚2h组),分别在给予盐酸溶液1h后,吸入2.4%七氟醚0.5h、1h、2h。于注入生理盐水或稀盐酸5h后处死各组大鼠,检测肺组织NF-кB活性及肺泡灌洗液中TNF-a、IL-6和IL-10的浓度。结果:与生理盐水组比较,盐酸吸入组肺组织NF-кB/p65阳性细胞增多(P<0.01),七氟醚处理1h与2h后阳性细胞较盐酸吸入组减少(P<0.05);盐酸吸入组肺泡灌洗液中TNF-a、IL-6和IL-10的浓度较对照组增高(P<0.01),七氟醚处理1h与2h后,与盐酸吸入组比较,TNF-a、IL-6的浓度降低(P<0.05),IL-10的浓度无明显改变。结论:七氟醚对酸吸入急性肺损伤可起部分保护作用,其机制与抑制NF-кB在大鼠肺组织中激活有关。Objective:To observe the effect of sevoflurane on hydrochloric acid aspiration induced acute lung injury in rats. Methods:80rats were randomly divided into 5 groups(n=15):NS group,HCL group,NS(1m L/kg) or HCL(1m L/kg)was instilled in rats airway.Sevoflurane groups(Sevo 0.5 h group,Sevo 1 h group,Sevo 2 h group):One hour after HCL instillation,rats received 2.4% sevoflurane inhalation respectively for thirty minutes,one hour or two hours.Five hours after NS or HCL instillation,all groups were exsanguinated.The nurmber of NF-кB positive cells in lung tissue and the concentration of TNF-a,IL-6,IL-10 in BALF were measured. Results:Compared with the NS group,after HCL instillation,the nurmber of NF-кB positive cells and the concentration of TNF-a,IL-6,IL-10 in BALF significantly increased(P0.01). Compared with the HCL group,in sevoflurane groups(Sevo 1 h group and Sevo 2 h group),the nurmber of NF-кB positive cells and the concentration of TNF-a,IL-6 in BALF decreased(P0.05). Conclusion:Sevoflurane could attenuate lung inflammation in rats with hydrochloric acid induced acute lung injury,inhibit activity of NF-кB and possess potential protective effects.
