摘  要:目的:探讨标本配穴改善心肌缺血的腧穴配伍效应及实现保护心肌缺血作用的基因调控途径。方法:将70只SPF级Wistar雄性大鼠根据随机数字表法分为正常组、模型组、LY294002组、IGF-1组、内关组、标本配穴组、标本配穴+LY294002组,每组10只。正常组注射等剂量0.9%NaCl溶液,余组采用盐酸异丙肾上腺素(ISO)大鼠腹部皮下注射的方法制作心肌缺血模型。于造模前开始干预,LY294002组和IGF-1组分别给予LY294002溶液和IGF-1溶液注射,共14d。内关组电针"内关"干预,连接HAN-200型电针仪,每次10min;标本配穴组电针"内关""足三里""关元",连接HAN-200型电针仪,每次10min;标本配穴+LY294002组,LY294002水溶液给药14d,造模前即给予电针"内关""足三里""关元",均每日1次,共持续21d。所有针刺在造模前进行电针针刺预处理。检测干预前后大鼠心率(HR)及ST段电压的变化,采用HE染色观察各组大鼠心肌组织病理形态学的变化,并检测心肌PI3K、Akt mRNA表达。结果:造模后,各干预组大鼠心率、ST段电压均高于正常组(均P<0.01);干预后,标本配穴组、IGF-1组和内关组心率、ST段电压较模型组改善(P<0.01,P<0.05),以标本配穴组、IGF-1组心率、ST段电压改善最明显(P<0.01)。心肌组织病理形态学方面,模型组心肌缺血明显,LY294002组最重,标本配穴+LY294002组介于两者之间;干预后,IGF-1组、内关组及标本配穴组心肌病理损伤明显改善,且IGF-1组和标本配穴组心肌组织病理损伤较内关组改善明显。PI3K、Akt mRNA表达方面,与正常组比较,造模后各组PI3K mRNA表达升高(P<0.01,P<0.05),IGF-1组与标本配穴组升高最明显(均P<0.01);LY294002组和标本配穴+LY294002组Akt mRNA表达降低(均P<0.01,P<0.05),余组Akt mRNA表达均升高(P<0.01,P<0.05),IGF-1组与标本配穴组Akt mRNA表达升高更明显(均P<0.01)。结论:标本配穴可改善慢性心肌缺血大鼠心率、ST段电压变化,减轻心肌组织病�Objective To explore the effects of principal-subordinate acupoints combination on improving myo- cardial ischemia, and the gene regulatory pathways for the protection of myocardial ischemia. Methods According to the random number table method, 70 SPF Wistar male rats were divided into a normal group, a model group, a LY294002 group, an insulin-like growth factors-1 (IGF-1) group, a Neiguan group, an acupoint combination group and an acupoint cornbination+LY294002 group, 10 rats in each one. Rats in the normal group were injected with 0.9% NaC1 solution, while rats in the remaining groups were treated with abdominal subcutaneous injection of iso- proterenol hydrochloride to establish the rat model of myocardial ischemia. Rats in tile I.Y294002 group and IGF-1 group were treated with injection of LY294002 solution and IGF-1 solution for 14 days. Rats in the Neiguan group were treated with electroacupuncture (EA) at "Neiguan" (PC 6) by using Han-200 EA apparatus for 10 rain per treatment. Rats in the acupoint combination group were treated with EA at "Neiguan" (PC 6), "Zusanli" (ST 36) and "Guanyuan" (CV 4) by using Han-200 EA apparatus for 10 rain per treatment. Rats in the acupoint combina- tion+ LY294002 group were treated with LY294002 solution for 14 days, and EA at "Neiguan" (PC 6), "Zusan- li" (ST 36) and "Guanyuan" (CV 4) was given before model establishment, once a day for 21 days. F.A pretreat- ment was given before model establishment in all acupuncture groups. The heart rate (HR) and ST segment volt- age were detected before and after treatment; the myocardial pathological morphology was observed hy HE stai ning; the expressions of PI3K mRNA and Akt mRNA were tested. Results After modeling, tlR and ST segment voltage in all intervention groups were higher than those in the normal group (all P〈0.01 ) ; after the interven tion, the HR and the ST segment voltage in the acupoint combination group, IGF-1 group and IGF-1 group were
