过敏性鼻炎对鼻息肉术后疗效的影响评估

摘　　要：目的探讨过敏性鼻炎对鼻息肉术后疗效的影响及其免疫病理机制。方法随机选取2012年8月—2014年9月该院耳鼻咽喉科收治的90例鼻息肉患者,鼻息肉合并过敏性鼻炎者40例,单纯性鼻炎患者50例,鼻腔粘膜组织正常者40例,采用免疫组化SP染色法和2R染色法检测鼻息肉患者和鼻腔粘膜组织正常者的Ig E阳性表达阳性表达和Eos浸润情况,对比鼻息肉患者和鼻腔粘膜组织正常者标本染色结果、Ig E阳性表达率、Eos浸润水平,统计手术随访结果。结果单纯性鼻息肉患者的治疗总有效率、Ig E阳性表达率、Eos浸润水平优于鼻腔粘膜组织正常者,差异有统计学意义(P<0.05);鼻息肉合并过敏性鼻炎患者的总Ig E阳性表达率、Eos浸润水平与单纯性鼻息肉患者、鼻腔粘膜组织正常者相比差异有统计学意义(P<0.05)。结论鼻息肉合并过敏性鼻炎会出现Ig E介导的反应性炎症,降低手术治疗效果,对鼻息肉手术疗效有不良影响。Objective To discuss the effects of postoperative curative effect of allergic rhinitis on nasal polyp and its immunologic pathogenesis. Methods 90 cases of nasal polyp patients treated in the nasen and ohrenheilkunde department of our hospital from August 2012 to September 2014 were selected, 40 cases were with nasal polyp associated with allergic rhinitis, 50 cases were with simple rhinitis and 40 cases were with normal nasal mucosa, the IgE positive expression and Eos infiltration of the nasal polyp patients and normal nasal mucosa patients were tested by immunohistochemical SP staining method and 2R staining method, the specimen staining results, IgE positive expression rates and Eos infiltration levels of the nasal polyp patients and normal nasal mucosa patients were compared, the follow-up results after operation were given statistics. Results The total treatment effective rate, IgE positive expression rate, Eos infiltration level of the simple nasal polyp patients were better than those of the normal nasal mucosa patients（P〈0.05）, the difference was statistically significant; compared with those of the simple nasal polyp patients and normal nasal mucosa patients, the differences in total IgE positive expression rate and Eos infiltration level of the nasal polyp associated with allergic rhinitis patients were obvious with statistical significance（P〈0.05）. Conclusion Nasal polyp associated with allergic rhinitis can lead to the appearance of reactive inflammation mediated by IgE, decrease the treatment effect of operation and has an adverse effect on the curative effect of nasal polyp operation.
