重症监护室中PICC置管的临床护理分析

摘　　要：目的探讨重症监护室中PICC置管的临床护理效果,丰富临床治疗经验。方法整群选择2013年6月—2015年10月该院收治的重症患者64例,将其随机分成两组,各32例,对照组采取常规护理,观察组在PICC置管后采取系统护理。对比两组护理效果。结果观察组保存时间是(45.7±4.6)d,对照组保存时间是(21.8±5.1)d;观察组未出现不良反应,对照组感染6例和外渗3例,观察组临床护理效果与对照组相比,差异有统计学意义(P<0.01)。观察组患者护理总满意度是96.9%,显著优于对照组的78.1%,差异有统计学意义(P<0.01)。结论选取PICC置管治疗重症患者后对患者采取系统护理,可以有效的降低不良反应的出现几率,提高护理满意度,确保抢救效果,具有显著的临床护理效果,值得临床进一步推广使用。Objective To investigate and analyze the clinical care for PICC replacement in the intensive care unit so as to accumulate the clinical treatment experience. Methods 64 critically ill patients admitted in our hospital from June 2013 to October 2015 were selected and randomly divided into two groups, the control group and the observation group with 32 cases in each. After PICC replacement, patients in the control group were treated by the usual care, while those in the observation group were given systematic care. And the nursing efficacy of the two groups was compared. Results The catheter retention time was（45.7±4.6） d in the observation group, and（21.8 ± 5.1） d in the control group; no complications occurred in the observation group, but in the control group, 6 cases had infection and 3 cases had extravasation, which showed that the difference in the clinical effect between the two groups was statistically significant（P〈0.01）. The overall nursing satisfaction was significantly higher in the observation group than that in the control group（96.9% vs 78.1%） with statistically significant difference（P〈0.01）. Conclusion Systematic care for PICC replacement in critically ill patients can effectively reduce the probability of postoperative complications, improve the level of nursing satisfaction, ensure the rescue effect with significant clinical nursing effect, therefore it is worthy of further clinical application and promotion.
