有限内固定结合外固定支架治疗严重Pilon骨折手术探讨

摘　　要：目的研究严重Pilon骨折在临床治疗中,行有限内固定结合外固定支架方案的临床效果。方法将该院2012年1月—2015年1月收治的严重Pilon骨折患者64例作为研究对象,随机分为对照组及观察组两个组别,每组32例。其中对照组采用切开复位内固定治疗,观察组采用有限内固定结合外固定支架治疗。观察两组患者的临床治疗效果,比较关节恢复疗效和并发症情况。结果两组患者在手术时间、骨折恢复时间上差异无统计学意义(P>0.05)。观察组关节恢复优良30例(93.8%),高于对照组的24例(75.0%);患者并发症出现1例(3.1%),低于对照组的6例(18.8%),P<0.05,差异有统计学意义。结论对于严重Pilon骨折患者而言,有限内固定结合外固定支架方案效果确切,能够减少并发症发生,提高踝关节恢复功能,具有简单安全的特点,值得临床推广。Objective To research the clinical effect of the plan of finite internal fixation combined with external fixation stand in treatment of severe Pilon fractures. Methods 64 cases of patients with severe Pilon fractures admitted and treated in our hospital from January 2012 to January 2015 were regarded as the research object and randomly divided into two groups with 32 cases in each, the control group were treated with open reduction and internal fixation, the observation group were treated with finite internal fixation combined with external fixation stand, the clinical treatment effect of the two groups was observed, the curative effect of joint recovery and complications of the two groups were compared. Results The differences in the operation time and fracture recovery time between the two groups were not big without statistical significance,（P〈0.05）, the excellent and good rate of joint recovery in the observation group was higher than that in the control group（93.8% vs 75.0%）（30 cases vs 24 cases）, the incidence of complication in the observation group was lower than that in the control group,（3.1% vs 18.8%）（1 case vs 6 cases）, and the differences were statistically significant P〈0.05. Conclusion The effect of the plan of finite internal fixation combined with external fixation stand for patients with severe Pilon fractures is definite, which can reduce the occurrence of the complication and improve the ankle joint recovery function with the characteristics of simple and security, and it is worth clinical promotion.
