PPH联合外切整形术治疗重度混合痔的临床疗效

摘　　要：为探讨吻合器痔上黏膜环切术（PPH）联合外切整形术治疗重度混合痔的临床疗效，本研究将2012年5月至2014年3月收治的80例重度混合痔患者按数字表法分为试验组和对照组，各40例，试验组行PPH联合外切整形术治疗，对照组行Milligan-Morgan术治疗，比较2组患者疗效。结果显示，2组虽在手术时间、术后尿潴留方面比较差异无统计学意义，P〉0．05；但在术中出血量、住院时间、术后疼痛、肛缘水肿、便中带血、肛门分泌物、肛门狭窄、创面愈合时间方面，试验组均优于对照组，P〈0．05。结果表明，PPH联合外切整形术治疗重度混合痔，具有微创、美观且康复快等优点，是一种安全有效的手术方法，值得临床推广应用。This study was to investigate the clinical effect of PPH combined with external resection ＆ plasticity in the treatment of severe mixed hemorrhoids, enrolled the 80 cases of this hemorrhoids who admitted in author＇s hospital from 2012-05 to 2014-03, and randomly divided them into trial group and control one（40 cases for each） ;performed above combined surgery on the trial group,only Milligan-Morgan procedure on control one, then compared both group＇s effects.As results, in operative time, postoperative uroschesis there was no statistical difference（ P 〉0.05）, but in intraoperative bleeding volume, hospitalization time,and postoperative pain, anal edge edema, stool with blood, anus secretion, anus stenosis as well as healing time of wound-surface the trial group was all superior to the control one（ P 〈0.05）.Results show that above-mentioned combined surgery for severe mixed hemorrhoids has such merits as being minimally invasive,beautiful and of faster recovery, etc, thus could be a safe and effective procedure, worthy to be popularized to use clinically.
