原发性高血压患者血清25(OH)D3水平及其相关因素分析

摘　　要：目的探讨原发性高血压患者血清25(OH)D3水平及其相关因素分析。方法选取原发性高血压患者200例(高血压组)和健康对照者200例(对照组),同时根据血压水平将高血压组分为1级高血压、2级高血压、3级高血压等3个亚组,采用全自动生化仪检测生化指标,酶联免疫吸附实验检测血清25(OH)D3水平,测量收缩压、舒张压、脉压、平均动脉压、脉压指数、体质指数、腰围、腰臀比等指标,分析血清25(OH)D3水平与血压指标的相关性。结果与对照组比较,高血压组在体质指数、腰围、腰臀比、收缩压、舒张压、脉压、平均动脉压、脉压指数、同型半胱氨酸、吸烟史、高血压家族史等方面均明显升高,25(OH)D3水平明显降低,同时随着高血压分级的升高,患者血清25(OH)D3水平逐渐降低。进一步直线相关分析结果显示:高血压患者血清25(OH)D3水平与收缩压、舒张压、脉压、平均动脉压、脉压指数均呈明显负相关(r分别为-0.718、-0.549、-0.478、-0.596、-0.804,均P<0.05或P<0.01)。结论原发性高血压患者血清25(OH)D3明显降低,其水平与收缩压、舒张压、脉压、平均动脉压、脉压指数等指标均呈明显负相关,对高血压严重程度有一定的预测价值。Objective To investigate the levels of serum 25-hydroxyvitamin D3 in patients with Essential Hypertension and its relevant factors analysis. Methods A total of 200 patients with essential hypertension（ EH group）,200 people with normal blood pressure（ control group） were selected for the study. Meanwhile,the total200 patients were divided into three groups according to their hypertension levels. The biochemical indicators were detected by automatic biochemical detector. The levels of serum 25-hydroxyvitamin D3 were detected with Enzyme linked immunosorbent assay. Parameters such as systolic blood pressure, diastolic blood pressure, pulse pressure,mean arterial pressure,pulse pressure index,body mass index,waist circumference,waist-hip ratio.The correlation of serum 25-hydroxyvitamin D3 with each hypertension parameters were analyzed.Results Compared with control group,all the body mass index,waist circumference,waist-hip ratio,systolic blood pressure, diastolic blood pressure, pulse pressure, mean arterial pressure, pulse pressure index,homocysteine,smoking history,family hypertension history were significantly increased,but the levels of serum25-hydroxyvitamin D3 were significantly decreased. With the blood pressure levels increased,but the levels of serum 25-hydroxyvitamin D3 were significantly decreased. Correlation analysis revealed that the levels of serum25-hydroxyvitamin D3 in the patients with essential hypertension was negatively correlated with systolic blood pressure,diastolic blood pressure,pulse pressure,mean arterial pressure,pulse pressure index（ r =- 0. 718,- 0. 549,- 0. 478,- 0. 596,- 0. 804,P〈 0. 05 or P〈 0. 01）. Conclusions The levels of serum 25-hydroxyvitamin D3 in the patients with essential hypertension was significantly decreased,and it was negatively correlated with the systolic blood pressure,diastolic blood pressure,pulse pressure,mean arterial pressure,pulse pressure index.
