探讨贝叶斯在中医针灸临床设计及评价中的应用

摘　　要：探索符合针灸学科特点,遵循中医基本理论的针灸临床设计及评价的新方法。本研究分析随机对照试验在中医针灸临床应用中面临的困难,分析中医基本理论与贝叶斯理论之间的联系。研究发现现行以随机对照双盲试验为塔尖的传统循证医学研究思路具有一定的局限性,不符合中医针灸学科的特点,违背了中医"天人合一"的系统观及"以人为本"的个体化治疗理念。贝叶斯符合中医针灸学科特点,可为中医针灸临床设计及评价体系的建立提供方法学参考。The paper aims to probe a new method of the clinical design and evaluation of acupuncture and moxibustion according to the characteristics of acupuncture and moxibustion and the basic theory of TCM. Analyze the difficulties of applying randomized controlled trial in clinical acupuncture and moxibustion and the relationship between the basic theory of traditional Chinese medicine（ TCM） and Bayesian theory. The research method of traditional evidence- based medicine,which takes randomized double blind test as the highest level of evidence,has some limitations,being inconsistent with the characteristic of acupuncture and moxibustion,violating the systematic perspective of ＂syncretism between heaven and man ＂of TCM and the people- oriented individualized treatment concept. Bayesian theory conforms to the characteristics of acupuncture and moxibustion and can provide methodological reference for establishing acupuncture and moxibustion clinical design and evaluation system.
