摘 要:目的研究比较在重型创伤性脑损伤(TBI)患者的诊断过程中,正电子发射计算机断层显像(PET)/CT和CT的诊断价值。方法回顾性分析21例车祸致重型TBI的非手术患者的临床资料,对比评价PET/CT和CT在重型TBI伤后的脑组织形态结构和功能代谢状态的显像作用效果。结果经影像学检查后,21例患者中,采用PET/CT检查发现阳性病灶[脑挫裂伤和(或)脑缺血]78.25处,明显多于使用CT检查所示阳性病灶61.25处,PET/CT检查阳性病灶部位敏感度高于CT检查,差异有统计学意义(P<0.01);经PET/CT检测出的大脑皮层和(或)皮层下区域阳性病灶部位及脑深部区域阳性病灶部位敏感度均高于CT检测出的病灶,差异有统计学意义(P<0.01)。结论 PET/CT对重型TBI患者较CT的临床诊断价值更高。Objective To research and compare diagnostic value of positron emission computed tomography(PET)/CT and CT in diagnosis of severe traumatic brain injury(TBI) patients. Methods A retrospective analysis was made on clinical data of 21 nonsurgical patients with severe TBI. Comparative evaluation was made on imaging effects between PET/CT and CT in brain tissue form structure and functional metabolic state after severe TBI. Results After imaging examination, PET/CT detected more positive lesion [cerebral contusion and(or) cerebral ischemia] as 78.25 parts than 61.25 parts by CT. PET/CT showed higher sensitivity in detection of positive lesion than CT, and the difference had statistical significance(P〈0.01). PET/CT also showed higher sensitivity in detection of cerebral cortex and(or) subcortical region positive lesion and cerebral deep positive lesion than CT, and their difference had statistical significance(P〈0.01). Conclusion PET/CT provides higher clinical value in clinical diagnosis of severe TBI patients than CT.
