摘  要:目的 总结Stanford B型主动脉创伤的最佳治疗方法及临床效果。方法 对我院2005年1月至2015年9月共治疗25例主动脉创伤临床资料进行回顾性分析,其中男性19例,女性6例,年龄21-60岁,平均35岁。18例采用覆膜支架腔内隔绝术治疗;7例采用传统开胸手术治疗。结果 18例采用覆膜支架腔内隔绝术的所有患者无急性脊髓缺血、无支架移位,无脑梗及脏器缺血等并发症,1例术后即刻造影示Ⅰ型内漏,1例术后即刻造影示Ⅱ型内漏。7例采用传统开胸手术治疗的患者中,2例1年后出现假性动脉瘤;1例术后胸腔持续引出陈旧血性液持续3周;1例胸腔感染为脓胸;1例手术后截瘫。结论 覆膜支架腔内隔绝术是治疗Stanford B型主动脉外伤的首选治疗方法,安全、可靠,创伤小、并发症少,目前已逐步取代开胸手术。Objective To summarize the best treatment method and clinical effect for Stanford type B aortic injuries. Methods In our hospital from January 2005 to September 2015, clinical data of 25 cases of aortic injury were analyzed retrospectively, 19 cases of male, female 6 cases, aged 21 to 60, 35 years old on average. For the treatment of 18 cases with coated stent lumen isolation, 7 cases with traditional open heart surgery. Results 18 cases accepting coated stent lumen isolation technique of all patients had no acute spinal cord is- chemia and stent displacement, no complications such as cerebral infarction and viscera ischemia, 1 case imaging showed immediate postoperative I type of leakage, 1 case showed lI type of leakage. 7 cases accept with traditional thoracotomy surgical treatment, 2 cases after 1 years appeared false aneurysm, 1 case appeared dark hemorrhagic fluid derivation for 3 weeks, 1 cases appeared pleural infection for pyothorax, 1 cases appeared paraplegia after the operation. Conclusion The coated stent lumen isolation technique is the first choice for treatment of Stanford type B aortic injury treatment,and it is safe, reliable, small trauma and fewer complicationsis now gradu- ally replacing open thoracic surgery.
