老年慢性心力衰竭患者临床特点

摘　　要：目的分析老年慢性心力衰竭患者的临床特征。方法选择2010年7月至2014年12月确诊的老年慢性心衰住院患者78例，根据年龄分为普通老年组和高龄老年组。收集患者一般资料、入院后检查结果及药物治疗情况，包括年龄，有无心衰相关性疾病（高血压、糖尿病、冠心病、肾功能不全），入院时心率、血压，入院后血肌酐、电解质，胸部X线片有无肺淤血，超声心动图指标，住院期间血浆BNP水平变化及药物治疗情况（主要指β受体阻滞剂、ACEI或者ARB、强心剂、利尿剂等）。住院期间给予标准化抗心衰治疗并在出院前评估心功能，检测血浆BNP水平。结果普通老年患者抗心衰治疗后心功能分级Ⅰ＋Ⅱ级为26％，心功能Ⅲ级为63％，心功能Ⅳ级为11％；而高龄老年患者则分别为21％、60％和19％。出院时BNP水平高龄老年组为（267．7±106．2）pg／ml，普通老年组为（401．5±98．9）pg／ml，两组比较差异有统计学意义（P〈0．05）。结论高龄老年患者抗心衰治疗后心功能分级及BNP水平高于普通老年患者。Objective To investigate the characteristics of chronic heart failure in the elderly. Methods Select 78 elderly cases, all data were taken from the hospitalized chronic heart failure cases from July 2010 to December 2014, and divide into 2 groups according the ages： elderly group and oldest elderly group. The following general information were collected, which including test results and drug treatment, age, with or without heart failure correlation disease（high blood pressure, diabetes, coronary heart disease, renal insufficiency）, hospitalized heart rate, blood pressure, serum ereatinine, electrolyte, chest radiograph with or without pulmonary congestion, ultrasonic cardiogram index, blood plasma BNP level before discharge and drug treatment in hospital（mainly refers to the beta blockers, ACEI or ARB, cardiac, diuretics, etc. ）. While in hospital, patients were given standard anti-heart failure treatment and blood plasma BNP （brain natriuretic peptide） level was measured before leaving hospital （P〈0.05）. Results After standard anti-heart failure treatment before discharge the percentage of Cardiac function grade one plus two, three and four classification were 26%, 63% and 11% respeetively in elderly group, corresponding 21%, 60% and 19% in oldest elderly group （P〈0.05）. BNP level were （267.7±106.2）pg/ml and （401.5±98.9）pg/ml in two groups （P〈0.05）. Conclusion Classification Cardiac function grade and BNP level in oldest elderly group is higher than elderly group after anti-heart failure treatment.
