摘  要:目的探讨血压变异性(BPV)对心房颤动发生及左心结构的影响。方法随机选取合肥市第三人民医院心内科2014年6月至2015年10月就诊患者120例,其中永久性房颤40例、持续性房颤40例、阵发性房颇40例;选取同期住院的窦性心律患者40例。检测24h动态心电图、动态血压、超声心动图,比较四组患者血压变异性及左房内径。结果心房颤动组的血压变异性、左房内径较正常对照组明显增加[(0.20±0.12)比(0.09±0.02),(45.0±2.48)mmt±(35.0±2.03)mm,P〈0.05];房颤组内比较,永久性房颤组较持续性房颤组、阵发性房颤组血压变异性明显增大(P〈0.05)。结论心房颤动患者血压变异性明显增加,且心房颤动的严重程度与血压变异性呈正相关。Objective To investigate the effects of blood pressure variability (BPV) on atrial fibrillation and left ventricular structure. Methods 120 patients with atrial fibrillation seeing doctor in Forty Third People's Hospital of Hefei from June 2014 to October 2015 were enrolled, including 40 cases of permanent atrial fibrillation, persistent atrial fibrillation 40 cases, 40 cases of paroxysmal atrial fibrillation; 40 cases of hospitalized patients in sinus rhythm were enrolled as control group. 24 h Holter monitoring, ambulatory blood pressure and echocardiography were measured in all patients, and the blood pressure variability and left atrial diameter were compared among four groups. Results In atrial fibrillation group, blood pressure variability, left atrial diameter increased significantly compared with the control group[(0.20±0.12) vs (0.09±0.02), (45.0±2.48)mm vs (35.0± 2.03 )mm, P〈0.05 ]; comparison group within the AF, the blood pressure variability was higher in permanent AF group than the persistent AF group and paroxysmal atrial fibrillation (P〈0.05). Conclusion The blood pressure variability is significantly increase in patients with atrial fibrillation and severity of atrial fibrillation and blood pres- sure variability were positively correlated.
