摘 要:目的 探讨超声下多种成像模式联合诊断乳腺结节的价值.方法 运用彩色多普勒血流显像(CDFI)、声触诊组织成像(VTI)、声触诊组织量化(VTQ)及超声造影(CEUS)等多种成像模式对261例患者的307枚乳腺结节进行检查并综合分析,最后与术后病理结果相比较.结果 307枚乳腺结节中,良性186枚,恶性121枚.CDFI诊断乳腺疾病的敏感度为71.2%,特异度为52.3%,准确度为73.4%;VTI评分诊断的敏感度为66.9%,特异度为41.4%,准确度为51.1%;VTQ剪切波速度(Vs)诊断的敏感度为83.5%,特异度为88.2%,准确度为86.3%;CEUS诊断的敏感度为85.1%,特异度为89.2%,准确度为87.6%;多模态联合应用诊断的敏感度为94.2%,特异度为95.7%,准确度为93.8%.CDFI、VTI、VTQ、CEUS及多模态联合5组诊断模式的曲线下面积分别为0.877、0.596、0.889、0.907、0.920,CDFI、VTI与VTQ以及CEUS之间差异具有统计学意义(P< 0.05);VTQ与CEUS之间差异无统计学意义(P>0.05).结论 多模态超声联合诊断可取长补短,明显提高对乳腺疾病的诊断符合率.Objective To explore the value of combined a variety of imaging modalities of ultrasound on diagnosis of breast nodules.Methods By color Doppler flow imaging (CDFI), virtual touch tissue imaging (VTI), virtual touch tissue quantification (VTQ), Contrast enhanced ultrasound (CEUS) and other imaging modalities, 307 breast lesions from 261 patients were detected.The results were analyzed and were compared with the postoperative pathological findings.Results 307 breast nodules included 186 benign and 121 vicious.The sensitivity, specificity and accuracy of CDFI on diagnosis of breast disease were 71.2 %, 52.3 % and 73.4 %, respectively, those of VTI were 66.9 %, 41.4 % and 51.1%, respectively, those of VTQ shear wave velocity Vs were 83.5 %, 88.2 % and 86.3 %, respectively, those of CEUS were 85.1%, 89.2 % and 87.6 %, respectively, and those of combined variety of imaging modalities were 94.2 %, 95.7 % and 93.8 %, respectively.The area under the curve of CDFI, VTI, VTQ, CEUS and combined multi-modal group diagnostic model were 0.877, 0.596, 0.889, 0.907 and 0.920, respectively.Among of CDFI, VTI, VTQ and CEUS, there were statistical differences (P 〈 0.05), while the difference between VTQ and CEUS had no statistical significance (P 〉 0.05).Conclusion Combined multi-modal ultrasound detection is complementary to improve the diagnostic accuracy on breast diseases.
