摘 要:目的探讨腹壁皮下寄养法治疗手指指尖(甲半月以远)完全离断伤的手术方法和临床疗效,为手指末节甲半月以远断指再植时,遇到无吻合血管条件的情况下,提供有效处理方法。方法自2013年2月-2014年12月,采用腹壁皮下寄养法治疗因各种外伤所致的手指末节甲半月以远离断伤、血管吻合条件较差或无血管吻合条件的9例10指。结果本组9例10指,8指完全成活,2指部分成活。其中8例9指获6个月随访,感觉恢复达s1~S3。结论在末节断指再植中,尤其是末节甲半月以远完全离断,遇到血管吻合条件较差或无血管吻合条件的情况下,可采用腹壁皮下寄养法,为无血管吻合条件的手指末节甲半月以远离断伤提供一种有效方法。Objective To explore the surgical approach and clinical efficacy of subcutaneous pocket procedure for the amputated fingertip (extending from the tip to the lunula) when microsurgical replantation is not feasible. Methods From February 2013 to November 2014, we treated ten fingers from nine patients using subcutaneous pocket procedure. All patients had sustained complete fingertip amputation across or distal to the lunula in digits. Microsurgical replantation was not feasible for all of the amputations. Results Eight fingers survived completely. Two fingers developed partial loss. Nine fingers were followed up for six months, and were classified as S1-S3 in the sensory evaluation. Conclusion When microsurgical replantation is not feasible, subcutaneous pocket procedure is a simple, reliable operation for digital amputation extending from the tip to the lunula.
