摘  要:目的探讨胸背动脉穿支皮瓣修复小儿足踝皮肤软组织缺损的临床疗效。方法对11例小儿足踝皮肤软组织缺损应用胸背动脉穿支皮瓣修复,显微镜下分离胸背动脉主干与胸背神经,保护胸背神经及背阔肌不受损伤,形成单纯的穿支皮瓣,皮瓣切取面积6.0cm×13.0cm~8.0cm×19.0cm。结果本组皮瓣术后全部成活,平均随访8.5个月。受区修复外形满意,供区直接缝合,背阔肌的功能保留完整。结论胸背动脉穿支皮瓣最大限度减少了供区的损伤;皮瓣较薄,克服了传统背阔肌肌皮瓣过分臃肿的缺点,是修复小儿足踝皮肤软组织缺损的理想皮瓣。Objective To investigate clinical curative effect of thoracodorsal artery perforator flap for repairing soft tissue defect of ankle and foot in children. Methods Free thoracodorsal artery perforator flap for repairing soft tissue defect of ankle and foot in 11 children, process free main thoracodorsal artery under operation microscope, thoracodorsal nerve and latissimus dorsi muscle were free from hurt during the free flap procedure, and simple perforator flap was formed. The flap size ranged from 6.0 cm×13.0 cm to 8.0 cm×l9.0 cm. Results All the perforator flaps survived with satisfactory results. The function of latissimus dorsi was well preserved postoperatively, and donor structure was directly sutureed. Conclusion The thoracodorsal artery perforator flap is characterized by the thinness of tissue and minimal sacrifice of donor structure with the preservation of thoracodorsal nerve and latissimus dorsi, which is an ideal flap for repairing soft tissue defect of foot and ankle skin and soft tissue defect in children.
