拇指桡背侧窄蒂逆行岛状皮瓣的临床应用

摘　　要：目的探讨拇指桡背侧窄蒂逆行岛状皮瓣修复拇指远端软组织缺损的方法及疗效。方法2008年9月-2012年12月，我们在拇指桡背侧逆行岛状筋膜蒂皮瓣的基础上进行改良，进一步缩窄筋膜蒂，修复拇指远端软组织缺损26例。术中切取2．0cm×2．5cm～3．0cm×3．5cm大小拇指桡背侧窄蒂逆行岛状皮瓣修复缺损。供区全部直接缝合。结果术后26例皮瓣全部成活。供、受区伤口均I期愈合。21例获随访，随访时间6～8个月，皮瓣颜色与周围皮肤相似，质地柔软，外观饱满，无臃肿。皮瓣感觉S1～S3，指间关节活动良好。结论拇指桡背侧窄蒂逆行岛状皮瓣易于切取，血运丰富，为拇指远端软组织缺损的修复提供了一种方便、有效的手术方法。Objective To investigate the method and effect of narrow pedicled reverse dorsordial flap for repairing the soft tissue defects in the distal of the thumb. Methods 26 patients were treated from September 2008 to December 2012 with narrow pedided reverse dorsordial flap. The areas of the flaps are 2.0 cm×2.5 cm～3.0 cm×3.5 cm. The flaps were raised from the dorsordial metacarpal region. Results All the 26 flaps survived. Followed up for 6 to 8 months of 21 cases. The color, shapes, texture and sensory of the flaps were well. Conclusion The operation of narrow peclicled reverse dorsordial flap is easy, the blood supply of this flap is abundant. It is a well approach to repair soft tissue defects of the distal end of thumb.
