VSD在Ⅲ型胫骨开放性骨折中的疗效分析

摘　　要：目的探讨封闭负压引流技术（vacuumsealingdrainage，VSD）在治疗Ⅲ型胫骨开放性骨折中的效果。方法选取2012年1月-2014年7月，收治的52例Ⅲ型胫骨开放性骨折患者．按照治疗方法的差异，分为实验组和对照组，每组26例。实验组26例，均彻底清创，骨折端复位，经过外固定后采用VSD治疗；对照组26例采用清创外固定术治疗，术后常规换药，比较两组的疗效。结果实验组随访6～12个月，住院时间（16．2±2．1）d，创面闭合时间（9．1±1．3）d，骨折愈合时间（209．8±13．9）d．均短于对照组（P〈0．05）；实验组发生骨髓炎、关节肿痛以及创面水肿等并发症的几率低于对照组（P〈0．05）。结论VSD在Ⅲ型胫骨开放性骨折治疗中，能够迅速稳定骨折端内环境，有效处理软组织损伤，术后骨折愈合时间、创面闭合时间短，降低并发症发生几率，有效提高了治疗效果，值得临床推广。Objective To investigate the vacuum-assisted closure effect in the treatment of type Ⅲ open fracture of the tibia. Methods From January 2012 to July 2014, inour hospital, 52 cases of typeⅢ open tibial fractures were, divided into experimental and in the control group, 26 cases in each group. 26 cases of the experimental group were debridement, fracture reduction, external fixation using VSD after treatment, the control group of 26 cases with external fixation for the treatment debridement, after conventional dressing, compare the efficacy of the two groups. Results The patients were followed up for 6-12 months, the experimental group of hospital stay （16.2 ± 2.1） d, wound closure time （9.1± 1.3） d, fracture healing time （209.8± 13.9）, were shorter than the control group （P 〈0.05）; less occurrence of osteomyelitis, joint swelling and wound edema and other complications the experimental group than that of control group （P 〈0.05）. Conclusion VSD can quickly stabilize the fracture, effectivly treat soft tissue injury, shorter postoperative healing time, wound closure time, reduce the chance of complications, improve the treatment effect, worthy of popularization.
