膝关节镜下前交叉韧带重建术对患者血骨桥蛋白和CD44表达的影响

摘　　要：目的探讨膝关节镜下前交叉韧带重建术对于机体骨桥蛋白（OPN）和CD44表达的影响。方法2013年2月至2015年6月选择在我院诊治的关节前交叉韧带损伤患者96例，根据随机数字表法分为治疗组与对照组各48例，两组患者均采用膝关节镜下手术治疗，对照组采用单隧道重建手术，治疗组采用双隧道重建手术。结果治疗组与对照组术后3个月的IKDC评分分别为（92．44±5．13）分和（85．25±4．67）分，均明显低于术前的（57．24±4．19）分和（57．10±4．87）分（P均〈0．05），同时术后组间比较差异也有统计学意义（P〈0．05）。治疗组术后总并发症发生率（6．3％）明显少于对照组（35．4％）（χ2=5．838，P〈0．05）。治疗组患者手术前、后OPN分别为（31．98±4．21）、（7．13±2．89）μg／L，CD44分别为（419．45±20．45）、（167．24±18．46）μg/L，两项指标手术前、后比较差异均有统计学意义（t值分别为34．214、21．774，P均〈0．05）。对照组患者手术前、后OPN分别为（32．09±3．98）、（15．98±3．67）μg／L，CD44分别为（421．87±19．45）、（245．98±20．56）μg／L，两项指标手术前、后比较差异均有统计学意义（t值分别为21．833、14．099，P均〈0．05）。两组患者手术后OPN、CD44比较差异均有统计学意义（t值分别为12．847、7．498，P均〈0．05）。结论相对于单隧道重建手术，膝关节镜下前交叉韧带双隧道重建术能促使膝关节功能的恢复，减少术后并发症的发生，其作用机制可能与有效降低外周血OPN与CD44表达含量有关。Objective To investigate the effect of arthroscopic reconstruction of anterior cruciate ligament on the expression of serum osteopontin （OPN） and CD44. Methods Ninety-six cases patients with joint cross ligament injury who were teated in Taihe Hospital Affiliated to Hubei University of Medicine from February 2013 to June 2015 were selected and randomly equally divided into treatment group and control group. Two groups were adopted arthroscopic surgical treatment, and the control group were given the single- tunnel reconstruction, the treatment group were given the double-tunnel reconstruction. Results The IKDC scores of 3 months after operation in the treatment group and control group were （ 92. 44 ± 5.13 ） points and （ 85.25±4. 67） points, both significantly lower than before operation （ （ 57.24 ± 4. 19 ） points, （ 57. 10± 4. 87 ） points;P〈 0. 05 ））, while there was statistically significant difference between groups （P〈0.05）. The total complication rate after 3 months operation in the treatment group was 6. 3%, significantly less than that of the control group （ 35.4% ; χ2 = 5. 838,P〈0. 05）.
