甲状腺术后低钠血症43例分析

摘　　要：探讨甲状腺术后低钠血症发生的原因、程度及相关因素。回顾性分析400例甲状腺术后患者的血钠水平与所用止血药物之间的关系。分析其中43例发生低钠血症的患者的严重程度与性别、年龄、手术范围、疾病的良恶性、去氨加压素的使用、术后低钙血症、术后低钾血症、术后甲状腺功能减退等因素之间的关系。结果显示,使用去氨加压素出现术后低钠血症的发生率明显高于使用蛇毒血凝酶及未使用止血药物的发生率(P<0.05);发生术后低钠血症的患者中,使用去氨加压素出现中重度低钠血症的概率明显高于非使用去氨加压素组(P<0.05),甲状腺术后低钠血症的严重程度与患者的性别、年龄、手术范围、疾病的良恶性、术后低钙血症、术后低钾血症、术后甲状腺功能减退等因素无显著性相关(P>0.05)。因此,甲状腺术后使用去氨加压素可能诱使低钠血症的发生并加重低钠血症的严重程度。To investigate the causes, severity and related factors of hyponatremia after thyroid surgery, a retrospective analysis of between the serum sodium level after the thyroid surgery and the hemostatic drugs in 400 patients was taken. Another retrospective analysis between the severity of postoperative hyponatremia and the sex, age, operating field, pathological diagnosis, hemostatic drugs, postoperative hypocalcemia, postoperative hypokalemia, post-operative hypothyroidism in 43 patients of hyponatremia after thyroid surgery was taken either. The incidence of hyponatremia in patients taking desmopressin after surgery was significantly higher than that in patients taking haemocoagulase and the patients without any hemostatic drugs （P〈0. 05） . The incidence of moderate and severe hyponatremia in patients taking desmopressin after surgery was significantly higher than that in patients without taking desmopressin （P〈0.05）. There was no significant difference between the severity of postoperative hyponatremia and the sex. age, operating field, pathological diagnosis, postoperative hypocalcemia, postoperative hypokalemia, post-operative hypothyroidism （P〉 0.05）. Therefore,the use of the desmopressin may increase the occurrence of hyponatremia and aggravate the severity of hyponatremia in patients after thyroid surgery,
