吴鞠通“精确识证”观念在乳糜尿治疗中的运用

摘　　要：以吴鞠通"精确识证"观念指导乳糜尿治疗,在尚未形成较大结块时可以按照王氏《乳糜尿中医证治》中的证型进行治疗,伴有乳糜血尿,则需要进一步采取止血措施,以免引起乳糜结块。若结块较大不能从尿道排出,应进行膀胱镜下冲洗取出结块,再大时可采用电切镜下Ellik膀胱冲洗器冲洗出乳糜血块,只有在乳糜结块较小,尿道尚通畅的情况下使用中医药辨证治疗才能取得较好的疗效。饮食调理要增加蛋白和中链脂肪酸的摄入,以进食鸡蛋清和瘦牛肉为主。The idea of WU Jutong＇s＂Precise Identification Evidence＂is summarized,and only upholds to accurately understand of the symptoms,disease process,diseases and a variety of evidence can improve the clinical efficacy. Guided by the concept of＂Precise Identification Evidence ＂to treatment chyluria,it can be treated according to Wang＇s symptoms in chyluria TCM treatment when the larger agglomerates has not formed. If accompanied by chylous Hemotochyluria,you will need to take more hemostatic in order to avoid agglomeration. If larger lumps can＇t discharge from urethral,Bladder microscope should be used to remove lumps and bladder electric mirror and Ellik Bladder flushing device should be used to flush out the more large lumps of chyle and blood. Only in small chyle lumps and urethra opening,Differential Treatment in Chinese medicine can achieve better results. Protein diet conditioning and medium chain fatty acid should be increased,which is more lean beef and egg white.
