缩短平均住院日对某肿瘤医院医疗质量影响的综合评价

摘　　要：目的对某三甲肿瘤医院实行合理缩短平均住院日的改革措施前后的医院医疗质量进行综合评价。方法根据某三甲肿瘤医院2010年-2014年的医院统计报表数据,选择相关的医疗质量指标,采用TOPSIS法对工作效率和医疗质量进行综合评价。结果医院于2014年实施合理缩短平均住院日的改革后,2014年平均住院日13.24天,为近5年最低。同时,4个医院工作效率的指标值为近5年最高,分别是:出院人次为33234人次、手术人次为5455人次、病床周转次数为33.23次和床位使用率120.17%;2个医院医疗质量的指标值为近5年最优,分别是:治愈好转率98.03%,为近5年最高;患者死亡率0.54%,为近5年最低。采用TOPSIS法评价结果显示2014年医疗质量为近5年最佳,Ci值为0.9894。结论缩短平均住院日不仅可以提高医院的医疗效率,帮助缓解医疗使用空间不足的困境,同时可以带动医院医疗质量的提高,实现患者、医院、社会三方共赢。Objective Comprehensively evaluate the medical quality after taken series of scientific and proper measures to shortened the average length of stay（ALOS） in some three-grade cancer hospital. Methods Ten indexes of medical quality data of some three-grade cancer hospital from 2010 to 2014 were selected and evaluated by using TOPSIS method.Results After taken series of measures to shortened ALOS in 2014, ALOS in 2014 was 13.24,which was the lowest in recent 5 years.At the same time, four indicators of hospital medical efficiency, discharged patients（33234）,man-time operation（5455）, bed utilization ratio（120.17%）and turn-over rate of beds discharge（33.23）,were highest in recent 5 years.For the indicators of hospital medical quality, treatment recovery rate（98.03%）was highest in recent 5 years,and mortality（0.54%）was lowest in recent 5 years.The evaluation results of TOPSIS method show that medical quality of 2014 is best in recent 5 years, the Ci value is 0.9895.Conclusion Shorten ALOS not only can help to improve the efficiency of hospital medical treatment, alleviate the dilemma of medical use of space shortage, but also can improve the medical quality of hospital,which can obtain wins of patients,hospitals and social.
