摘  要:目的探讨CT阈值法软件测量乳突气房体积的可行性,分析乳突气化程度与面神经管垂直段(VFN)位置的相关性。资料与方法回顾性分析39例行64层螺旋CT颞骨平扫的患者(共59侧),运用CT值阈值法软件测量乳突气房体积,根据乳突气房气化程度将患者分为气化良好组、气化不良组及无气化组,比较男性与女性、左侧与右侧的乳突气房体积,测量并比较3组VFN至外耳道骨性后壁的距离(d1)、圆窗龛的距离(d2)及乳突外侧缘骨质的距离(d3)。结果气化良好组23侧,气化不良组20侧,无气化组16侧。男性与女性乳突气房体积差异无统计学意义(P>0.05);左侧与右侧突气房体积差异无统计学意义(P>0.05)。气化良好组d1、d2小于气化不良组(P<0.05),d3大于气化不良组(P>0.05);气化不良组d1、d3与无气化组比较,差异有统计学意义(P<0.05),d2差异无统计学意义(P>0.05);气化良好组d1、d2、d3与无气化组比较,差异均有统计学意义(P<0.05)。结论 CT阈值法可准确地测量乳突气房的体积,可作为判断乳突气房气化程度的新方法。乳突气房气化程度越好,VFN位置越靠近圆窗龛及外耳道骨性后壁,临床上依据乳突气房气化程度可评估VFN位置情况。Purpose To explore the feasibility of CT threshold value method in measuring the volume of mastoid air cell, and to analyze its relationship with the location of vertical facial nerve(VFN). Materials and Methods Thirty-nine patients receiving 64-slice spiral CT were retrospectively analyzed. The volume of mastoid air cell were measured using the threshold value software. Patients were divided into three groups based on the degree of pneumatization of mastoid air cell: good, fair and none. The volume of mastoid air cell was compared in males and females, left side and right side. The distance from the posterior wall of external auditory meatus osseous(d1), round window niche(d2) and lateral margin of mastoid bone(d3) to VFN of three groups were compared. Results There were 23 sides with good pneumatization, 20 sides with fair pneumatization, and 16 sides with none pneumatization. There was no significant difference between males and females(P〈0.05) or between left side and right side(P〈0.05). The d1 and d2 of good pneumatization group were less than that of fair pneumatization group(P〈0.05), but d3 was greater than that of fair pneumatization group(P〈0.05). There were significant differences in d1 and d3 between fair and none pneumatization groups(P〈0.05), while no significant difference in d2(P〈0.05). The d1, d2 and d3 between good and none pneumatization groups were significantly different(P〈0.05). Conclusion The threshold value method with CT attenuation value correctly measures the volume of mastoid air cell. There is a certain relationship between the degree of mastoid air cell pneumatization and the location of VFN.
