摘  要:目的左心室内的血流特征对于心脏功能的病理生理研究具有重要意义,本研究主要探讨黑血对比序列双反转恢复快速自旋回波序列(DIR-FSE)中心内膜下及心室腔内血流高信号的诊断意义。资料与方法选取2015年3-12月于解放军总医院放射诊断科行心脏MRI检查的48例患者作为观察组,并纳入32例健康志愿者作为对照组。观察两组DIR-FSE中出现高信号的情况,并分析患者的临床资料及MRI图像,分析这种高信号的出现与肌小梁结构的多少、心腔形态、心肌运动、瓣膜功能的相关性。结果观察组48例患者中,38例出现高信号,10例未见高信号。38例出现高信号患者中,心室腔内高信号22例,附壁高信号7例,心尖段高信号9例。对照组32例健康志愿者中,1例出现附壁高信号,18例出现心尖段高信号,13例未见高信号。观察组高信号的出现率高于对照组,差异有统计学意义(P<0.05)。结论 DIR-FSE中血流高信号可以作为心室血流异常的征象,对心脏疾病的诊断有一定的提示意义,有进一步研究的必要。Purpose The turbulent flow pattern in the left ventricle cavity is of great significance to reveal the pathophysiology of cardiac function. In the study we aims to investigate the diagnostic value of the high signal from intraventricular flow on double inversion recovery fast spin echo sequence(DIR-FSE). Materials and Methods Fortyeight patients and 32 healthy volunteers had routine cardiac MRI examinations and were enrolled in the study. The frequency and pattern of flow-induced intraventricular high signal on the DIR-FSE were correlated with the ventricular trabeculae, the ventricular contour, myocardial motion and the valves function. Results The intraventicular high signal could be classified as intracavity, mural, apical region. 38 of the 48 patients had intraventricular high signal on DIR-FSE. Of them, 22 were intracavity, 7 were mural and 9 were apical. On the 32 healthy controls, 1 had mural and 18 had apical high signal. The frequency of high signal on patients were different than the controls where P〈0.05. Conclusion The high signal from intraventricular flow on DIR-FSE is a sign of abnormal blood flow patterns. It may be a marker of cardiac dysfunction and need further investigation.
