全髋关节置换术患者医院感染的干预措施研究

摘　　要：目的探讨全髋关节置换术后医院感染的发生情况及相关危险因素,并提出相应的预防干预措施,为临床治疗与护理提供参考依据。方法选取2005年1月-2015年6月医院行全髋关节置换术的患者398例,探讨全髋关节置换术后医院感染的发生情况及相关危险因素,对行全髋关节置换术后医院感染发生率及危险因素进行分析,数据采用SPSS 18.0软件进行统计分析。结果共发生感染8例患者,感染率2.01%;分离出病原菌15株,以革兰阴性菌为主,共10株占66.67%;多因素logistic回归分析显示,合并基础疾病、手术时间、术前血红蛋白、年龄、长期激素应用史为全髋关节置换术后医院感染的独立危险因素(P<0.05)。结论全髋关节置换术后引发医院感染的因素较多,应制定相应的干预措施,以预防和控制医院感染的发生。OBJECTIVE To investigate the incidence and risk factors of hospital infections after total hip replacement,and to put forward the corresponding preventive measures so as to provide reference for clinical treatment and nursing.METHODS Totally 398 patients who underwent total hip replacement in Jan.2015 to Jun.2015 were selected to explore the incidence and related risk factors of hospital infections after total hip replacement.The incidence rate of hospital infections and the risk factors of hospital infections were analyzed.Data were statistically analyzed by software SPSS 18.0.RESULTS Totally 8patients had infections,and the infection rate was 2.01%.A total of 15 pathogens were detected,including 10strains（66.67%）of gram negative bacteria as the dominant strains.The multivariate logisticregression analysis showed that the independent risk factors of hospital infections after total hip replacement were underlying diseases,surgical time,preoperative hemoglobin,age and long-term history of hormone application（P〈0.05）.CONCLUSION There are many factors that cause hospital infections after total hip replacement,and the corresponding intervention measures should be developed to prevent and control the occurrence of hospital infections.
