摘　　要：目的探讨藻酸盐银离子预防肿瘤患者经外周静脉穿刺中心静脉置管(PICC)穿刺部位感染的临床效果,为预防PICC置管局部感染提供参考依据。方法选择2013年1月-2015年9月医院收治的肿瘤且行PICC置管138例患者为研究对象,随机分为观察组和对照组,各69例;观察组在PICC置管成功后,采用藻酸盐银离子辅料覆盖穿刺点,并用3M敷贴固定,对照组在PICC置管成功后,采用无菌纱布覆盖穿刺点,并用3M敷贴固定;观察两组患者穿刺部位感染和不良反应,数据采用SPSS 13.0软件进行统计分析。结果观察组穿刺部位感染率为0,对照组穿刺部位感染5例,感染率为7.2%;观察组皮肤过敏和静脉炎发生率均明显低于对照组;观察组自感舒适率明显高于对照组,两组比较,差异均有统计学意义(P<0.05)。结论藻酸盐银离子预防肿瘤患者PICC置管局部感染的临床疗效显著,可有效预防穿刺部位感染,降低皮肤过敏和静脉炎发生风险,提高患者局部舒适度,值得临床应用。OBJECTIVE To investigate the clinical effect of alginate in prevention of PICC catheter insertion sites infections in tumor patients,and to provide reference for prevention of PICC catheter insertion site infections.METHODS A total of 138 tumor patients treated with PICC in our hospital were chosen from Jan.2013 to Sep.2015 as subjects,who were randomly divided into observation group and control group（69cases in every group）.In observation group,patients were treated with alginate combined with 3Mtransparent stickers after PICC,and patients in control group were treated with gauze combined with 3Mtransparent stickers after PICC.The PICC catheter insertion site infections and adverse reactions were observed and compared between the two groups.Data were analyzed with SPSS 13.0.RESULTS There was no PICC catheter insertion site infection in observation group,and there were 5cases with PICC catheter insertion site infections in control group with the infection rate of 7.2%.The rate of skin allergies and phlebitis in observation group was significantly lower than that in control group,and the comfortable rate in observation group was significantly higher than in control group（P〈0.05）.CONCLUSIONThere′s significant clinical effect of PICC in tumor patients＇ catheter insertion site infections.It is effective to prevent PICC catheter insertion site infections,decrease skin allergies and phlebitis,increase local comfortableness,and is worthwhile for clinical application.
