经食道三维超声心动图在26例二尖瓣成形术中的临床价值

摘　　要：目的评价经食道三维超声心动图在26例二尖瓣成形术中的临床价值。方法 26例因二尖瓣反流接受二尖瓣成形术的心脏病患者,男性20例,女性6例,采集术前、术中RT-3D-TEE图像,将术前所见与术中发现进行比较,并在术中即刻评价手术效果。结果通过对26例患者术中所见二尖瓣装置病变情况与2D-TTE和3D-TEE观察结果进行对比,3D-TEE对二尖瓣装置病变的诊断较2D-TTE更接近术中所见,其中,3D-TEE对22例二尖瓣脱垂亚区的诊断更准确。3例患者经术中TEE评价后,二尖瓣成形术改行二尖瓣人工机械瓣膜植入术。结论经食道三维超声用于二尖瓣成形术的术前评估及术中监测,可以及时解决手术中的问题,预测手术效果,提高手术成功率,在二尖瓣成形术中具有重要的临床价值。Objective To evaluate the clinical value of 3D transesophageal echocardiography（3D- TEE） in 26 patients of mitral valve repair. Methods In this study,26 patients in need of mitral valve repair were used, of which include 20 males and 6 females. ALL patients were examined by 3D- TEE before and during the operation. The surgical processes were monitored. Results Based on 26 cases of patients seen in the mitral apparatus lesions compared between 2D- TTE and 3D- TEE observations, 3D- TEE in diagnosis of mitral apparatus lesions is closer to see during the operation than 2D- TTE. 3D- TEE is more accurate than 2D- TTE in the diagnosis of sub regions of 22 cases of mitral valve prolapse. The 3 patients of mitral valve repair converted to mitral valve replacement, after intraoperative TEE evaluation. Conclusion The application of TEE in mitral valve repair resolves surgical problems timely, predicts surgical outcome, and enhances surgical success rate. It is of important clinical value in mitral valve repair.
