摘 要:目的通过对惠州地区无偿献血者抗-Mur及Mur抗原进行检测,了解其在人群中的分布,建立本地区的Mur血型检索库。方法分别采用高通量微板法和微柱凝胶法对献血者进行抗-Mur筛查和确认,并对其Mur抗原进行血清型检测。根据检测结果及献血者相关资料,建立Mur血型检索库,指导临床用血及献血者招募。结果总筛查人数32 305人,抗体阳性104人,阳性率0.32%,其中41例具有临床意义,男女间的差异具有统计意义(P<0.05),女性高于男性;抗原阳性1 862人,阳性率为5.76%。在近1年的临床输血应用中,共避免41袋含有抗-Mur血液流入临床,并为5例抗-Mur阳性患者找到相合血液。结论 Mur血型检索库的建立,能有效避免含有Mur抗体的血液制品流入临床,减少因此引起的疑难配血及输血不良反应,并快速找到相合血源,同时有利于稀有血型献血者的保留和电子配血的开展及推广。Objective To establish a red blood ceil retrieval library for solely Mur blood type by detecting the anti-Mur and Mur antigen in blood donors of Huizhou district and analyzing its distribution. Methods Microplate polybrene method was used to screen anti-Mur and DG Get Coombs cards method was used to confirm. Microplate brine method was used to de- tect the serum type of Mur antigen, A Mur blood type retrieval library had been established and was used to guide clinical blood transfusion and recruitment of blood donors according to test results and related information of the blood donors. Re- sUits A total of 32 305 blood donors were screened, 104 of whom were antibody positive at a positive rate of 0. 32%. A- mong all the donors, 41 showed clinical significance. The differences between men and women showed statistical significance ( P 〈 0. 05). From the result, the positive rate of women was found to be higher than men. Besides, 1 862 people were anti- gen positive at a rate of 5.76%. In almost a whole year, 41 bags of blood and blood products containing anti-Mur were pre- vented from circulating to the hospital. Five patients with anti-Mur accepted the matched blood in time from the Mur blood type retrieval library. Conclusion The Mur blood type retrieval library can effectively avoid blood products with Mur anti- bodies circulating into the hospital, suggesting that it can facilitate finding matched blood, reduce complicated operation and adverse reactions of blood transfusion. Not only will it will help to store the data of rare blood donors, but it will also confer benefits to the development and promotion of electronic blood crossmatching.
