腹腔镜与经腹宫颈癌根治术320例临床分析

摘　　要：目的分析腹腔镜与传统开腹广泛子宫切除及盆腔淋巴结清扫术治疗宫颈癌的临床资料,探讨其可行性、安全性及并发症。方法回顾性分析西安市第四医院2008年1月至2012年1月320例行腹腔镜与开腹广泛子宫切除及盆腔淋巴结清扫术治疗早期宫颈癌患者的临床资料,其中行腹腔镜150例,开腹170例。结果腹腔镜组手术时间[(182±52.3)min]、术中出血量[(377±91.9)m L]、胃肠功能恢复时间[(1.9±0.3)d]均少于开腹手术组[(220±69.5)min、(865±147.5)m L、(2.8±1.2)d](P<0.05);两组切除的淋巴结数差异无统计学意义(P>0.05);两组并发症发生率差异有统计学意义(P<0.05)。结论腹腔镜下广泛全子宫切除加盆腔淋巴结清扫术治疗早期宫颈癌临床效果肯定,与开腹手术治愈率相近,具有创伤小,恢复快,并发症发生率低等优势。Objective To analyze the clinical data of cervical cancer patients treated by laparoscopic and abdominal radical hysterectomy and abdominopelvic lymphadenectomy,and to discuss the feasibility,safety and complications of two methods. Methods The data of 320 patients with cervical cancer hospitalized in Xi＇an Fourth Hospital from January 2008 to January 2012 who underwent laparoscopic or abdominal radical hysterectomy and abdominopelvic lymphadenectomy were retrospectively analyzed,in which 150 cases underwent laparoscopic,and 170 cases underwent abdominal radical hysterectomy and abdominopelvic lymphadenectomy. Results The operation time[（ 182 ± 52. 3） min],the blood loss in operation[（ 377 ± 91. 9） m L],gastrointestinal function recovery time [（ 1. 9± 0. 3） d]of laparoscopy group were significantly less than those of open surgery group[（ 220 ± 69. 5） min、（ 865 ± 147. 5） m L、（ 2. 8 ±1. 2） d ]（ P〈0. 05）. But there was no significant difference in the number of lymph node excision between two groups（ P〉0. 05）.The operation complication rate of two groups was significantly different（ P〈0. 05）. Conclusion Laparoscopic radical hysterectomy and pelvic lymphadenectomy is an effective method for treating early cervical cancer,having a quick postoperative recovery,with small trauma,faster recovery,and lower complication rate.
