摘 要:目的探讨妊娠高脂血症患者血浆高密度脂蛋白(HDL)亚类的分布特征及相关脂酶的变化。方法采集2015年8-10月在中国人民解放军第202医院就诊的50例妊娠高脂血症患者和50例正常孕妇的血浆,采用Lipoprint脂蛋白亚类分析系统检测血浆HDL亚类组成和含量;Elisa法检测血浆肝脂酶(HPL)、脂蛋白脂肪酶(LPL)含量。结果妊娠高脂血症患者血浆大颗粒HDL浓度及百分比[(0.74±0.23)mmol/L、(39.43±6.71)%]明显低于正常孕妇[(0.96±0.28)mmol/L、(45.95±6.95)%],差异有统计学意义(P<0.05);中颗粒HDL百分比明显高于正常孕妇,差异有统计学意义(P<0.05);妊娠高脂血症患者血浆HPL、LPL浓度明显低于正常孕妇,差异有统计学意义(P<0.05)。结论妊娠高脂血症患者血浆HDL亚类分布特征的改变,即大颗粒HDL减少,较HDL浓度改变更具有临床指导意义。妊娠高脂血症患者血浆HPL、LPL可能作为临床检测妊娠高脂血症病情变化的新指标。Objective To discuss the distribution characteristics of HDL subclasses and the changes of related esterase in the pregnant patients with hyperlipidemia. Methods The blood plasma was collected respectively from 50 pregnant patients with hyperlipidemia and 50 healthy pregnant woman who were diagnosed and treated in the China People's Lib- eration Army 202 Hospital from August to October of 2015. Lipoprotein subclasses analysis system was used to detect the composition and content of HDL subclasses, and Elisa was used to test the content of HPL and LPL in blood plasma. Results The analysis results of HDL subclasses showed that the concentration and percentage of large particles HDL [ (0.74±0.23)retool/L, (39.43±6.71 )%] of the pregnant patients with hyperlipidemia in blood plasma was obviously lower than that of healthy pregnant woman[(0.96±0.28)retool/L, (45.95±6.95)%], the differences being of statistical signifi- cance (P 〈 0.05). The percentage of medium particles HDL of the pregnant patients with hyperlipidemia was apparently higher than that of healthy pregnant woman, the differences being statistically significant (P 〈 0.05). The HPL and LPL concentration of pregnant patients with hyperlipidemia was obviously lower than that of healthy pregnant woman, and the differences were of statistical significance (P 〈 0.05). Conclusions The changes of HDL subclasses distribution charac- teristics of the pregnant patients with hyperlipidemia, which is that the large particles HDL decreases, have more clinical significance than the changes of HDL concentration. The HPL and LPL in the blood plasma of pregnant patients with hy- perlipidemia might be regarded as the new index to monitor the changes of the disease.
