摘 要:目的探讨单独应用MR动态增强扫描(DCE)时间-信号强度曲线(TIC)、氢质子磁共振波谱(^1H-MRS)与二者联合应用对乳腺肿瘤的诊断价值.方法回顾性分析41例乳腺肿块(恶性肿瘤23例,良性肿瘤18例)的DCE-TIC和^1H-MRS表现,并与手术或穿刺后病理结果对比.所有患者均为女性,年龄17-69岁,平均(41.0±14.7)岁,均应用GE公司的1.5TSigna HDxt扫描仪,8通道乳腺专用线圈,扫描序列包括:轴位STIR、TIWI、乳腺容积动态增强序列(VIBRANT)和1 H-MRS扫描,评价单独应用DCE-MRI时间-信号强度曲线(TIC)、磁共振波谱(^1H-MRS)及二者联合应用对诊断乳腺肿瘤的诊断能力.结果 ①以DCE-TIC类型诊断乳腺肿瘤的敏感度为82.6%,特异度为77.8%,准确度为80.5%;②以^1H-MRS出现Cho峰诊断乳腺肿瘤的敏感度为70.0%,特异度为83.3%,准确度为75.6%;③将TIC与1 H-MRS Cho峰结合,诊断的敏感度为78.0%,特异度为88.9%,准确率为82.9%.结论 DCE-MRI TIC诊断乳腺肿瘤的特异度较低,联合应用DCE-TIC和^1H-MRS可提高乳腺肿瘤性病变的诊断特异度、准确度.Objective:To investigate the value of combined dynamic contrast-enhanced breast magnetic resonance scan(DCE-MRI)time-intensity curves with ^1H magnetic resonance spectroscopy(MRS)in the diagnosis of breast tumors.Methods:Retrospectively analyzed the DCE-TIC and ^1H-MRS findings of 41 cases with breast tumors,including 23 malignant cases and 18 benign cases,and compared with the pathological results after surgery or biopsy.All patients are women,17 to 69-year-old,average 41-yearold,All scanning was performed on a 1.5T GE Signa HDxt scanner,an eight-channel bilateral breast radiofrequency coil was used,scanning sequence includes short time inversion recovery(STIR)、TIWI、VIBRANT in the axial orientation and 1 H magnetic resonance spectroscopy(MRS).DCE-MRI TIC and 1 HMRS of all lesions were jointly analyzed in the identification of benign and malignant breast tumors.Results:The diagnostic sensitivity,specificity and accuracy rate of TIC alone was 82.6%,77.8%and80.5% respectively.Magnetic resonance spectroscopy(MRS)of diagnosis sensitivity of breast cancer is70.0%,specificity is 83.3%,accuracy is 75.6%.The diagnostic sensitivity,specificity and accuracy rate for combined application of TIC and MRS was 78.0%,88.9% and 82.9%respectively.Condusion:The specificity of DCE-TIC in the diagrosis of breast tumors is low,and the combined application of DCE-TIC and′H-MRS can improve the diagrostic spectficty and accuracy of breast tumors.
