晚间持续应用BiPAP呼吸机辅助呼吸治疗Ⅱ型呼吸衰竭患者的疗效观察

摘　　要：目的探讨晚间持续应用Bi PAP呼吸机辅助呼吸治疗II型呼吸衰竭患者的疗效。方法选择50例我院收治的II型呼吸衰竭患者,行常规药物治疗,并采用Bi PAP呼吸机辅助治疗。按使用呼吸机的不同时间点分为B组和A组,其中B组24例,白天使用呼吸机,A组26例,晚间使用呼吸机。对两组患者治疗前后的各血气指标、生命体征参数以及临床疗效进行分析和对比。结果 1两组患者治疗后各时间点的p H值以及血氧分压均较治疗前均有明显上升,而二氧化碳分压均较治疗前有显著下降,差异具有统计学意义(均P<0.05);且A组患者治疗后p H值、二氧化碳分压、血氧分压均显著由于B组,差异具有统计学意义(均P<0.001);2两组患者治疗后各时间点的心率明显低于治疗前,呈下降趋势,而呼吸频率和平均动脉压则在治疗后24h出现上升,在治疗后72h和10d反而下降,差异具有统计学意义(均P<0.05);且A组患者治疗后各生命体征指标明显比B组要好,差异具有统计学意义(均P<0.001);3两组患者治疗后,A组患者的治疗效果明显优于B组,差异具有统计学意义(P<0.001)。结论对ICU监护的II型呼吸衰竭患者行晚间持续应用Bi PAP呼吸机辅助呼吸治疗可明显改善患者血气、生命体征,有效提高治疗的效果,值得临床借鉴和推广。Objective To explore the efficacy of continuous application of BiPAP ventilator assisted breathing in the treatment of patients with type II respiratory failure. Methods Selected 50 patients with type II respiratory failure in our hospital,treated with routine drug therapy,and used And BiPAP ventilator assisted treatment. Divided them into group B and group A according to the different time points of the use of breathing machine,There were 24 cases in the group B,used breathing machine during the day,26 cases in group A,used breathing machine at night. Analyzed and compared the blood gas indicators,vital signs parameters and clinical efficacy of the two groups of patients before and after treatment. Results①The pH value and oxygen partial pressure of each time point after treatment in the two groups were significantly increased compared with before treatment,but the carbon dioxide partial pressure were significantly decreased compared with before treatment,the difference was statistically significant（ all P〈0. 05）; ②The heart rate of two groups of patients at each time point after treatment was significantly lower than that before treatment,with downward trend,The respiratory rate and mean arterial pressure increased in 24 h after treatment,and then decreased in 72 h and 10 d after treatment,the difference was statistically significant（ all P〈0. 05）,and the vital signs of patients in group A after treatment were significantly better than the group B,the difference was statistically significant（ both P 〈0. 001）; ③ Two groups of patients after treatment,the treatment effect of group A was significantly better than that of group B,and the difference was statistically significant（ P 〈0. 001）. Conclusion Application of BiPAP ventilator assisted breathing in the treatment of patients with type II respiratory failure can significantly improved the blood gas,vital signs of patients,and improved the effectiveness of treatment effectively,which is worthy of clinical reference and promotion.
