摘　　要：目的:了解河南省人民医院(以下简称"我院")住院病房的退药现状,分析退药原因,有针对性地提出应对措施,保证患者用药安全,提高临床服务质量。方法:统计我院住院药房2014年9 594张退药处方,从退药原因、退药科室、退药品种等方面分析退药现状。结果:在9 594张退药处方中,退药原因以更改医嘱为主(占41.88%),其次是患者出院(占25.10%),再次是患者拒绝用药(占14.41%);退药科室主要是消化内科、呼吸内科和胃肠外科;各科室退药种类以抗菌药物为主,含有抗菌药物的处方占总退药处方的48.44%。结论:我院退药量较大,但临床退药现象不可避免,应加强医院内部管理制度,提高医、护、药工作人员的工作效率和服务水平;加强与患者沟通,提高患者用药的依从性;提高药学服务质量,促进临床合理用药。OBJECTIVE： To investigate the status of drugs return in inpatient ward of Henan People＇s Hospital（ hereinafter referred to as ＂our hospital＂）,and to analyze the reasons of drugs return,so as to propose the countermeasures to ensure safety and effectiveness medication for patients,and improve the medical quality in clinic.METHODS： 9 594 prescriptions of drugs return in inpatient pharmacy of our hospital in 2014 were statistically analyzed in terms of drugs return reasons,departments and varieties,etc. RESULTS： Of the 9 594 prescriptions of drugs return,the reasons of adjustment of prescriptions took the lead（ 41. 88%）,followed by the discharge of patients（ 25. 10%） and the rejection of patients（ 14. 41%）. The main departments were department of gastroenterology,respiratory medicine and gastrointestinal surgery. Meanwhile,the major withdrawn drugs were antibiotics,accounting for 48. 44% of the total prescriptions of drugs return. CONCLUSIONS： Apparently,the drugs return amount in our hospital is relatively large,yet the phenomenon of clinical drugs return is inevitable. It is necessary to strengthen the internal management of our hospital,improve the sense of responsibility and service level of medical staff and enhance communication to increase the patient＇s compliance with medication,so as to improve the medical quality and promote the rational drug use in clinic.
