临床护理路径在准分子激光角膜屈光手术中的应用

摘　　要：目的探讨准分子激光角膜屈光手术患者实施临床护理路径的效果。方法 2013年1月至2014年12月,行准分子激光角膜屈光手术患者1 708例,按入院时间分为两组,分为对照组患者689例,入院时间2013年1月-12月,实行常规护理;实验组患者1 019例,入院时间2014年1月-12月,实行临床护理路径。比较两组患者对健康教育知识的掌握情况,观察术后视觉效果、患者依从性及患者的满意度。结果实验组患者均遵医嘱定期复查,按时点药,依从性好,并发症发现早,无严重并发症;对照组67.9%患者不能完全遵循医嘱。裸眼视力（Uncorrected visual acuity UCVA）达到或超过术前最佳矫正视力（Best corrected visual acuity BCVA）者,实验组为（98.9%）;对照组（95.7%）。主观视觉质量调查对照组46%患者诉视物疲劳、眩光等不适;实验组25%。想把此手术推荐给自己的亲戚或朋友者,对照组患者63%;实验组患者90%。患者满意度实验组92.6%;对照组62.9%。健康教育达标率实验组96.3%;对照组74.1%。两组比较差异有显著意义（P〈0.05）。结论对准分子激光角膜屈光手术实施临床护理路径可提高患者健康教育知识的掌握程度,能提高患者的依从性,及时发现并发症,减少了严重并发症的发生,提高患者视觉质量及满意度。提高护理人员的自身素质及护理质量,有一定的经济效益和社会效益。Objective To investigate the effect to apply clinical nursing path to excimer laser corneal refractive surgeries.Methods Altogether 1,708 patients having received excimer laser corneal refractive surgery from Jan.2013 to Dec.2014 were selected.While the 689 patients given the surgery from Jan.to Dec.2013 being under regular nursing,the 1019 patients undergoing the surgery from Jan.to Dec.2014 were taken care with clinical nursing path.The postoperative visual effect and patients＇ compliance,satisfaction and knowledge of health of the two groups were compared.Results The patients of the experimental group followed well doctors＇ directions to receive periodical reexamination and take medicines on time with better compliance,their complications were found early,and no serious complication occurred.To the contrary,67.9%of the patients in the control group did not follow fully doctors＇ advices.As to the proportion of the patients with UCVA=BCVA,the experimental group was 98.9%and the control group 95.7%.For subjective visual quality,46%of the patients in the control group and 25%of the patients in the experimental group complained of fatigue and discomfort.While 63%of the patients in the control group wanting to recommend the surgery to their relatives or friends,90%of the patients in the experimental group were glad to make the recommendation.Patients＇ satisfaction was 92.6%for the experimental group and 62.9%for the control group.Health education success rate was 96.29%in the experimental group and 74.04%in the control group.The difference was statistically significant.Conclusions Clinical nursing path applied to the excimer laser corneal refractive surgery will improve the patients＇ health knowledge and compliance,benefit a timely notice of complications,improve quality of nursing staff and nursing quality and bring in considerable economic and social benefits.
