铜绿假单胞菌注射液术中胸腔留置在伴胸膜凹陷征肺癌患者中的应用

摘　　要：目的探索术中腔内注射铜绿假单胞菌注射液对伴有胸膜凹陷征肺癌患者的应用价值。方法前瞻性随机对照试验,瑞金医院普胸外科收治的肺部肿瘤患者59例随机分为胸腔内用药组(P组)29例和对照组(C组)30例,对比2组术前、术后第3日、术后第10日CD3、CD4、CD8水平、胸引管拔管时间及术后并发症和死亡率。结果 2组术前一般情况差异无统计学意义。P组术前和术后第3日、第10日CD8的变化差异无统计学意义(P>0.05),术后第10日CD3、CD4水平差异有统计学意义(P=0.023)。C组术前和术后第3日、第10日CD3、CD4、CD8的变化差异无统计学意义(P>0.05)。P组术后第10日CD3、CD4均高于C组(P=0.000,P=0.031);P组术后拔胸引管时间平均2.4 d,C组术后拔胸引管时间平均3.8 d,差异有统计学意义(P=0.036);2组术后并发症和死亡率差异无统计学意义(P>0.05)。结论术中腔内注射铜绿假单胞菌注射液安全,不良反应低,且可以通过细胞免疫调节,缩短术后胸引管拔管时间,从而改善患者生活质量。Objective To investigate the clinical value of intraoperative pleural infusion of Pseudomonas aeruginosa mannose sensitive hemagglutinin for lung cancer patients with pleural indentation. Methods Ac- cording to a prospective randomized principle, 59 cases of lung cancer patients were divided into a pleural infu- sion group （P group 29 patients） and a control group （C group 30 patients）. The preoperative, 3 d postoperative and 10 d postoperative CD3/CD4/CD8 levels, the detubation time, and postoperative morbidities and mortalities were compared. Results The baselines of both groups had no difference. The preoperative, 3 d postoperative, and 10 d postoperative CD3/CD8 of P group had no significant difference （P 〉 0.05）, while the preoperative and 10 d postoperative CD4 showed significant difference （P z 0.023）. C group presented no difference in CD3/CD4/ CD8 during the hospital stay. The 10 d postoperative CD3/CD4 level of P group was significantly higher than that of C group （P=0.000; P=0.031）. The average detubation time of P group was 2.4 d, significantly shorter than 3.8 d of C group （P=0.036）. The postoperative morbidities and mortalities had no significant differences in the 2 groups （P 〉 0.05）. Conclusion Intraoperative pleural infusion of PA-MHSA for lung cancer patients with pleural indentation is safe. It can shorten the detubation time by modulating cellular immunity, and improves the quality of life of patients.
